Camp Tadmor Paintball WAIVER AND RELEASE OF LIABILITY READ CAREFULLY THE UNDERSIGNED 1. Has asked Northwest Conservative Baptist Assoc./ Camp Tadmor to be allowed to play PAINTBALL and acknowledges that the game involves physical exertion and other risks, known and unknown, and may result in injury to the undersigned or others playing the game, even if all safety rules are followed. 2. Is aware of the possibility of risks of injury, including, but not limited to, being shot by a paintball, falling, tripping, being hurt by a wild animal, heat stroke, heart attack, insect bites & stings, poisonous, cutting, stinging or piercing plants, improper use, malfunction or operation of equipment by the undersigned or any other player and others not following the safety rules. Players will be exposed to both natural and man-made hazards. The possibility of permanent disability or death does exist. 3. Agrees to play the game according to the rules which have been explained, posted, and provided and to follow directions given by any game referees. 4. Warrants and acknowledges that his/her physical condition is excellent and his/her mental state is sufficiently stable to enable him/her to participate safely in the game. This includes not being on any medications that would pose a risk to him/her under physically exerting conditions. 5. Agrees to use any paintball equipment in a manner which will not cause injury or damage to himself / herself or others playing. 6. Authorizes use of photos, videos, name, comments, game results, etc. for promotional purposes. 7. Understands and agrees to follow all safety rules and procedures and that such rules and procedures not followed by others could result in my injury or death. 8. Understands that serious and permanent eye injury, including loss of eyesight, can occur if approved paintball safety eyewear is not worn in any area where paintball guns maybe intentionally or accidentally discharged. He/She understands that it his/her responsibility to wear approved safety eyewear and he/she accepts that responsibility. 9. Understands that the safety eyewear can fog or become dirty and agrees that despite any problems that he/she will keep them securely fastened to protect the eyes and will not remove them while on the playing field, at the chronograph, at the target range or in any other area where he/she might be struck by a paintball, other gas powered projectile device used in paintball. 10. Understands that loss of hearing from an ear shot, disorientation and injury from throat, groin, head or ear shots can occur if proper safety equipment is not worn to protect these areas, and understands it is his/her responsibility to wear or not wear such items. IN CONSIDERATION of being permitted to play the game, the undersigned, on behalf of himself/herself, his/her successors or assigns hereby releases and forever discharges Northwest Conservative Baptist Assoc./ Camp Tadmor, land owners, managers, equipment distributors, and their successors, assigns, employees, officers, agents and franchisees, dealers or operators both jointly and severally, from any and all actions, covenants, claims and demands for damages, costs, expenses (including attorneys' fees), loss or injury, however arising, including negligence of Northwest Conservative Baptist Assoc./ Camp Tadmor which may have been or may be sustained by the undersigned in any way relating to or arising out of the playing PAINTBALL or the equipment, including but not limited to, the manufacture, selection, delivery, possession, use, or operation of the equipment or the natural environment. The undersigned desires and agrees to assume any and all risks. I hereby agree to the above release and acknowledge the receipt of the referenced safety rules (see reverse side). I have read each and every item of this waiver, I understand what each item means, will participate in spite of the herein mentioned risks and I agree to abide by the terms of this waiver. This waiver applies to any and all paintball games I participate in at present or in the future. Signature ______________________________ Age ______ Event Date ___/___/___ Print Name _____________________________ Address _________________________ Signature of parent or guardian City _________ State ___ Zip ____ _______________________________ (If less than 18 yrs old) Phone ___________________________ Revised 12/15/2003 PAINTBALL RULES Masks • Masks MUST be on AT ALL TIMES from the time you get your gun to the time you turn it in • If masks are removed at any time, your paintball game is over Barrel Plugs • Keep barrel plugs in guns at all times except during game play Hits • If you are hit by any player and the ball breaks, call HIT loudly four times and walk off the field with your gun above your head without talking • If you are hit and are not sure if the ball broke, call PAINTCHECK loudly and the official will check you • If you are hit and the ball didn’t break, call CLEAN loudly • Do not aim for the head, but head shots DO count • Elbow to hand and knee to foot shots do NOT count Gun Safety • Keep guns pointed up in upright position at all times except during game play • If gun jams, hold gun over head and call for an official • Never look down the barrel to see if it is dirty. If you think your gun is clogged, call for an official to squeegee and check your gun • NEVER shoot at anyone who is not wearing approved eye protection • Velocity adjusting tools are not allowed on the playing field Conduct • Use sportsmanlike conduct at all times (especially honesty) • Do not climb trees • No shooting outside of the marked field • Players are disqualified if they go out-of-bounds • DO NOT shoot the officials or else your game will be over • While in play, your armbands must be worn on your arms, in plain view. No concealing or removing them. Paintballs • Use Tadmor field paint only • DO NOT pick up paintballs off the ground • When you run out of paintballs, your game is over Officials • Referees will be wearing bright orange vests and yellow face masks • Referees are in charge of the game—WHATEVER THEY SAY IS THE LAW • Do not change teams during game play Rule Violations • If a player violates a rule, the officials may decide that their game is over and their money will not be refunded By signing the WAIVER AND RELEASE OF LIABILITY form on the reverse side of this form I acknowledge that I have read, understand and agree to play by the above listed rules.
