Completing your Application Form
This sheet provides you with some additional guidance as you complete your application form.
In order to make the processing of your application as smooth as possible it is important that
you complete all sections of the form legibly (in block capitals) and accurately.
Please state the programme and module(s) (if appropriate) you wish to apply for.
o Stand Alone Module
o Stand Alone Module (attendance only)
o Certificate of Personal and Professional Development (CPPD in Cancer Nursing
Care, Chemotherapy or Radiotherapy).
o Diploma in Cancer Care (nurses only)
o BSc in Cancer Care (nurses or allied health professionals)
o Post-Graduate Diploma/MSc Nurse Practitioner: Cancer Care (nurses only)
If the programme has more than one module please indicate when you would like to
undertake each individual module including the title and the start date of each one
The closing date for applications is one month prior to their start date. After this date

o You can apply for a maximum of two in any one term.
o If you apply for two concurrent modules you cannot start a third module unless
both assignments have been submitted (not necessary to have results).
o If you have commenced one module but not yet submitted your assignment an
application to attend any additional module would be considered at the
Programme Leaders discretion.
o Multiple stand alone modules may not automatically be assumed to count
towards a CPPD, Diploma, Degree or Post Graduate Diploma. Please seek

Section 6 and 7
Please indicate who will pay your fees by ticking the appropriate box.
Your designated Trust representative must sign Section 7 if you are from a Trust eligible
for ‘contract’ funding. If you are unsure who this is please check with your Trust’s
education and training department or with the administrative staff at the Royal Marsden
School of Cancer Nursing and Rehabilitation.

If your employer is paying your fees, or if you are paying them yourself, an invoice will
be sent to your home address for you to arrange payment.
If you are applying for or have secured a grant to pay your fees please provide us with
details of how much you have been awarded and who to send the invoice to.
If you are an applicant from the Royal Marsden NHS Foundation Trust section 7 of the
form will be signed when all other sections have been completed and your form has
been sent to the Royal Marsden School of Cancer Nursing and Rehabilitation.
The Royal Marsden School of Cancer Nursing and Rehabilitation supports students with
learning difficulties and disabilities. To help us to provide appropriate support please
inform us of your needs.
Please list your present and previous posts in the last 5 years.
Remember to include a summary of your key duties.

This should include your Nurse or AHP pre registration education.

This statement is very important, as it is one of the main means by which the selector will
decide whether you are suitable to undertake the programme/ module you have applied for.
Please include not only your reasons for applying and what you hope to gain from the
module but also information on its relevance to your work.
For many modules in order to complete the assignment you must be working in an
appropriate clinical setting, please give some details of the clinical area in which you
work to enable the selector to make a decision.
It may be useful to get a friend or colleague to read this section through before
submitting your application to ensure that it reads well and provides a good overview of
your reasons for wishing to undertake the programme/ module you are applying for.
Please remember to sign and date your form. Also get your manager to sign and
date to confirm that they approve your application, that it is part of your Training
& Development Plan and your release from ward duties on all module dates.
Applications not signed by both you and your manager at Section 15 will be returned.
If you work at the Royal Marsden NHS Foundation Trust you will also need to complete
a Study Leave Form. Please enclose the fully completed and signed Study Leave
Request Form with your application – it will be considered for approval when your
course place is confirmed.
