I. Power on Phone
A. Locate large power button on top of the phone.
B. Hold the power button until the phone slightly vibrates. This will power on the phone.
Wait several seconds until the phone is ready to use.
II. Locate Applications
A. Press the center black button to open the gallery.
B. Locate the silver ring surrounding the black center button. This acts as a directional pad
to navigate the menu on the device. Pressing the top of the ring navigates up; pressing the
bottom of the ring navigates down, and so on. From here on we will be referring to these
directions as up, down, left, and right.
C. Press down; then press right.
Applications should be highlighted in the Gallery.
D. Press the black center button to select Applications.
E. Press down 3 times and you should find the WVSurvey Application selected:

III. Navigating the Forms
A. You can use the silver directional ring (left, right, up and down) to navigate to the form
you wish to access and fill out, then press the black center button to select it. Upon selecting
your desired form, use the silver ring buttons to navigate and fill out the Application.
B. Entering information
1. There are certain questions that require you to enter a word or a name. For
these questions, use the corresponding letters on the phone keypad to fill it out. The
order of the letters on each button corresponds to how many times you have to press
that button to enter that letter. For instance, pressing “7” four times will enter “S”, and
pressing “6” twice will enter “N”. To enter the word “hat”, you must press “4” twice,
press “2” once, then press “8” once.
2. Some questions require you to enter a number. For these questions, all you
need to do is press the corresponding numbers on the keypad of your phone.
3. Some questions require you to enter a date. The field will display “Enter Date”.
Press the center black button to enter the date select screen. The date is shown in the
format DD-MM-YYYY. Press the center black button to enter the calendar screen, then
use the silver directional buttons to navigate the calendar and enter the appropriate
4. Some questions require you to check one or more boxes. To select a box, use
the silver directional buttons to navigate to the box you want to select, then press the
black center button to check that box. When you are satisfied with the answer you have
selected, press the left blue button to access the Options menu then use the silver
directional buttons and the center black button to navigate to and select “Next”.
C. Canceling, deleting and returning to previous menus:
1. If at any point you need to delete a character or go back to something previous,
you can select the right blue button at the top of the phone device. Next to the button
on the screen you will see the word Delete.
2. If at any point that you want to save your progress on the phone, save and exit
your Application, or just exit your Application, you can select the left blue button at the
top of the phone device to access the Options menu.
D. Selection of options on the phone device
1. You must use the up or down silver directional ring buttons to navigate the
options menu, then you can use the black center button to select the option you have
E. Cancellation of Application progress
1. At any point you wish to cancel your Application or leave your Application in
progress, you can push the red button.
This brings up a screen that says, “Exit Application? WVSurvey”.
If you wish to exit, press the left blue button to select “Yes”.
If you don’t wish to exit the program, press the right blue button at the top of the phone
to select “No”.
F. Application complete
1. When you have finished filling all required information out you can do 1 of 3
things by using the up and down silver directional ring to select “Send Now”, “Send
Later”, or “Send to new server”:
2. Select “Send Now”. This will give you the option to send your completed form to
the database. A screen will appear that says, “Send text message to ##########?”
Select yes each time it asks until the entire form is sent.
3. If your phone does not have a connection to mobile phone service, an error
message will be displayed on the phone and the saved form data will be stored on the
phone. Press the black center button to select “OK”. When you have service, press the
left blue button to select Options, then press down four times until you reach “View

forms that haven’t been sent. Press the left blue button to enter the Options menu,
then press down until you reach “Send all unsent”, then press the black center button
to send all saved and unsent messages:
