    A waste form must be completed and accompany the waste each time a pickup request is made,
                                     or it will not be removed.

  EHS has revised and simplified the forms waste generators use to request a waste pickup. The forms serve
   several purposes. First, they allow you to notify EHS in writing of a request for waste pickup. Second, they
 document the person and lab that is requesting the pickup, and third, by signing, you certify that the information
               being provided to EHS is accurate, and that an effort has been made to minimize
                                     the amount of waste you are generating.

  If you choose to notify EHS of the need for a pickup by phone or e-mail, you must fill out the form and have it
                               ready with the waste when EHS arrives for pickup.
