									                             Instructions for Necessary Medical Forms

Grades 3, 5 and all New Students
Health Records Form – All students entering grades 3, 5 and all New Students need to complete this
       form and have it signed by a medical doctor. The school nurse uses this form to monitor
       compliance with the state laws.

              Complete the enclosed form
              Attach Immunization record
              Return by July 15, 2009


Grades 1, 2, 4 and 6
Immunization Records Update – (if applicable) All students grades 1, 2, 4 and 6 need to have
      up-to-date immunization records on file with the nurse. If your child has received an immunization
      within the past year, please send a copy of the updated immunization record to June McGee, RN.


Grades 7-12
      Annual Pre-Participation Physical Exam – to be completed by all students entering grades 7-12.
      This form must be completed and signed by a medical doctor. The physical must be dated within
      the calendar year (i.e. physical for 2009-2010 school year must be dated in 2009). Please be sure
      the athlete’s name is written on the top of the page.

Medical History Form – (on back of Annual Pre-Participation Physical Exam) to be completed by the parent
or guardian of all students entering grades 7-12.

              Complete the enclosed form
              Attach Immunization record
              Return by July 15, 2009


Medication Form (if applicable) The medication form gives the school nurse permission to distribute
      prescription medication to your student. If your student takes prescription medication of any kind,
      this form must be completed and signed by the parent or guardian and the physician.

              Have your child’s physician complete Medication Form
              Return form to June McGee, RN
