Please note the following:
   • This form must be filed in the Registration & Records Office along with the grade
       transmittal sheet or the incomplete grade will not be given
   • An incomplete grade contract must be signed by both the faculty member and the student
       at the time of final grade submittals.
   • The time allowed to complete the coursework is determined by the faculty member (a
       maximum of 60 days is permitted).
   • Grade changes must be submitted by the faculty member no later than the close of the
       long semester immediately following the term in which the incorrect or disputed grade
       was given.
