                                                   INSTRUCTIONS
The purpose of the Performance Appraisal is to rate the employee's performance during the last rating period
and establish, if necessary, developmental action plans for any improvement needed.

  1. To rate each item under each performance category, match the Rating Scale that most accurately reflects
     the employee's performance and write it in the space provided. (Space is provided for comments.)

  2. Calculate the average rating for each category as follows:
    a.   Add the performance standard ratings within each performance category.
    b.   Divide the sum of a. by the number of performance standards within the category.

  3. Calculate the employee's Overall Rating as follows:
    a.   Add all 24 of the performance standard ratings from all categories.
    b.   Divide the sum of a. by 24, the total performance standards from all categories.

  4. Complete the Developmental Action Plans, if necessary, prior to the review session for any improvement
     areas.

  5. Use the Supervisor's Comments section for documentation of overall ratings other than "Attains
     Expectations".

  6. Offer the employee the opportunity to complete the Employee's Comments section.

  7. Complete the Work Performance Objectives section.

  8. Complete the Salary Information on the front and obtain your supervisor's signature.
