					How to use Quality Improvement worksheet
The Quality Improvement worksheet can be viewed, printed, and saved as a Word document from the Yhplus.com website. There are specific QI worksheets for each clinical topic and a general worksheet under the Quality Improvement module. To view click on the link for QI worksheet under resources. Once open, the worksheet can be saved to your computer or be printed. A. Background Performing quality improvement (QI) projects in your practice helps you assess the care of your patients using evidence-based quality indicators. YHPlus offers QI projects for each clinical topic and a method for recording and reporting these QI projects:  to specialty boards to qualify for Maintenance of Certification Part IV  to health plans to satisfy requirements for QI projects or pay-for-performance  to obtain up to 20 CME credits for each QI project B. Using the QI Worksheet There is a similar format to each QI project: 1. The practice defines a QI project that is specific, measurable and within a scheduled timeframe. 2. The practice chooses a set of measures to be tracked over time. Each YHPlus QI project has suggested measures drawn from the educational material of the presentation. You can use the ones suggested or develop your own. 3. Baseline data from your practice is collected (retrospectively if you have an electronic health record or if you do not, you can do a chart abstract; or prospectively, as you see patients for the chronic illness you are trying to improve). 4. Look at the data in summary and list up to four ideas for improvement. 5. Choose one idea that can be tested in one week. Keep the change on a small scale. Document who will be doing what. 6. Repeat the process until the desired change is achieved. 7. Review performance to determine the impact of the change on patient care and staff. 8. Decide whether to implement into practice, re-test or re-design. 9. Repeat the cycle to add other ideas to test.

C. CME credit     For completing 1 through 4 above, up to 5 AMA PRA Category 1 credits™ will be awarded. For completing 5 through 9 above, up to 5 more AMA PRA Category 1 credits™ will be awarded. For completing the brief narrative and data charts from your QI project, 5 more AMA PRA Category 1 credits™ will be awarded. For completing one whole project, another 5 AMA PRA Category 1 credits™ are awarded for a total of 20 CME credits.

Submit the completed worksheet, or completed section of the worksheet, to IAFP for CME credit. Worksheets can be submitted by email, fax or mail. Typed information is preferred over handwritten information.
