									Welcome to Hebron Pediatric Practice!

We look forward to helping you in these formative years as you watch your child develop and grow. We
recognize our role in a patient’s health is relatively small in the overall scheme of things. Lifestyle, diet, sleep
and exercise patterns have a major impact on how you feel. We will be able to discuss these matters during
your visits. We appreciate the confidence you placed in us by choosing our practice. Our Practice Manager,
office staff and I will make every effort to be available to you.

The office is shared among 4 pediatric physicians: Dr. Brent Cochran, Dr. Jon Robin Voyles, Dr. Anthony
Schapker and Dr. Jessica López. In the absence of your primary physician, others will be available to cover any
problems in the office. The physicians are all board certified in Pediatrics. Our primary call group includes:

                                                 OFFICE HOURS

Office hours are from 8:45 a.m. to 4:30 p.m. Monday through Friday. Visits are by appointment. Our office
phone number is (812) 473-8986. Should you need to change or cancel any appointment, please make every
effort to call 24 hours in advance.

Pediatricians in the Evansville area have joined in a group called Tri-State Pediatrics, L.L.C., for the purpose of
providing an After Hours Office for urgent problems which arise after our office has closed in the evening and
on weekends. This is for acute illnesses, not routine exams or other problems that would be better handled in
our office during regular hours. We are 4 of the 14 pediatricians in this group, and 1 to 2 of us, along with
pediatric nursing staff, provide care in this office. The After Hours office is located in the Center for Children,
just off Washington Avenue in front of St. Mary’s Medical Center. The After Hours Office may be reached
from 4:30 p.m. to 9 p.m. weekdays and 9:00 a.m. to 6:00p.m. weekends and open most holidays. The After
Hours Office phone number is (812) 485-7780. For emergent issues please call 911 or go to your nearest
emergency room.


                                                 OFFICE PHONE

The office staff is prepared to answer most of your routine questions. The nurses working with each physician
along with our phone nurse are available during our regular office hours. Some questions may require
consultation with the physician. Our phone nurse will handle your acute care needs along with any general
questions. For more specific issues, behavioral consults and complex issues, the appropriate primary nurse will
respond to your call. Your call will be triaged and returned as soon as possible the same day.

Our office telephone, 812-473-8986 is answered 24 hours a day, with an answering service assisting during
lunch and after hours. Calls during the hours of the After Hours Office will be directed to the staff of that office.
For urgent or emergent problems during the night, the answering service will page the nurse on call. We make
every effort to return calls promptly; however, if your problem is truly emergent, go to the nearest Emergency
Room and have their staff call while care is being administered.


                                                 PRESCRIPTIONS

For refills on prescriptions, please have the pharmacy fax us a request and we will process these within 24
hours. Prescriptions such as controlled substances must be picked up by you or someone you have authorized.
A phone prescription line is available for each physician and requires 24 hours minimum notice for processing.
For pick up of any prescription, ID and signature are required.


                                                     PAYMENT

As a courtesy we have a billing department that will submit claims for services provided. Demographic
information and insurance will be verified on each visit to ensure we have accurate information. If your
insurance requires a co-pay, payment is expected at time of service. Our fees are comparable to area
pediatricians. Please feel free to discuss this important matter with our billing staff. For families that are self
pay, please ask about our payment plan. Please understand that immunizations, tests and other treatments
require additional charges depending on their cost and the time it takes to administer them. Some insurance
policies may not cover these items. Your financial obligation is to understand your policy benefits and
preferred providers.
