CONSULTING AGREEMENT
This Consulting Agreement (the “Agreement”) is entered into as of ________ ,20__ (the “Effective Date”) made between ____________ ("Client"), with a principal place of business at _______________[ADDRESS] and [NAME OF IC] ("Consultant"), with a principal place of business at _______________ [ADDRESS].
1.
