Summary of medical practice, expertise & professional interests:
Professional qualifications: Dr. Valentine qualified MB ChB at Leeds University in 1986. He became a member of the Royal College of Physicians of London (MRCP) in 1989 and a fellow of the Royal College of Anaesthetists (FRCA) in 1992 having passed the required examinations. He was granted Foundation Fellowship of the Faculty of Pain Medicine of the Royal College of Anaesthetists in October 2007. NHS Consultant appointments: Dr. Valentine was appointed as a “Consultant in Anaesthetics and Pain Management” at Norfolk & Norwich University Hospital NHS Trust at Norwich in October 1996. He has been a full-time pain management specialist since September 2001 when he gave up his anaesthetics commitment and extended his pain management role to include cancer pain management at the Priscilla Bacon Centre. Current NHS practice: Dr. Valentine‟s present NHS duties are divided equally between the management of chronic benign pain at the Norwich Pain Management Centre and the management of cancer related pain at the Priscilla Bacon Centre Specialist Palliative Care Unit, Norwich. The Norwich Pain Management Centre is a multidisciplinary unit encompassing specialist nurses, a clinical psychologist, physiotherapists & occupational therapists. He has extensive experience in the management of a wide range of painful conditions including those that can result from personal injury. This includes conditions such as complex regional pain syndromes (CRPS) and other neuropathic pain syndromes. Lumbar spinal pain & sciatica, thoracic spinal pain, cervical spinal pain & whiplash syndromes, shoulder pains and complex headaches. Private practice: Dr. Valentine‟s private consulting rooms are at the Spire Norwich Hospital and the Sandringham Hospital in King‟s Lynn. He has admitting rights to both hospitals, treating patients with a wide range of painful conditions primarily with traditional injection based interventions. Specialist medical interests: Dr. Valentine‟s medical interests include traditional thermal & pulsed radiofrequency techniques for spinal & nerve root pain and spinal cord stimulation for a range of chronic pain states. He also has experience in the management of long-term intrathecal drug delivery using a range of external & implanted drug-delivery pumps although he no longer implants these devices.

Educational & training roles: In addition to an active involvement with local training & education, Dr. Valentine has presented and chaired sessions at national meetings including those of the British Pain Society and the Neuromodulation Society of UK & Ireland. He has presented internationally at Royal Society of Medicine meetings in Italy & Romania and at a European Society of Anaesthesiologists Meeting in Paris. He has critically reviewed articles for publication in the British Journal of Anaesthesia, European Journal of Pain and “PAIN” all of which are widely respected international journals.

Medicolegal experience & training:
Training: Dr. Valentine attended the Professional Solutions “Medicolegal Report Writing” course in October 1998 and started writing medicolegal reports in March 1999. He completed the Bond Solon “Excellence in Report Writing” & “Courtroom Skills” courses in September 2005. Report writing experience: Dr. Valentine has now been preparing pain management medical reports for the Court for 9 years and has completed over 230 medical reports and statements in this time. He is currently preparing 1 - 2 medical reports each week. Many of his reports relate to claimants with symptoms of chronic pain persisting beyond the expected healing period. Not infrequently pain is associated with a higher level of disability than might be expected. His ratio of primary claimant, joint & defendant work over the last 2 year has been in the region of 45% claimant, 45% defendant and 10% joint instruction. His recent and past report writing statistics are available on his „pain-expert‟ website. Courtroom experience: Dr. Valentine has attended Court on 3 occasions and has given oral evidence on 2 occasions. He gave oral evidence at Cambridge County Court in September 2003 and at Norwich County Court in May 2007. Presentations to a medicolegal audience: In May 2008, Dr. Valentine presented on „The Medicolegal Aspects of Complex Regional Pain Syndrome (CRPS)‟ at the „Kennedys Liability Seminar‟ in London. He is currently booked to present at the Oxford Medicolegal Society Meeting in February 2009.

Positions of responsibility:
Dr. Valentine was secretary of the Neuromodulation Society of UK & Ireland (2002 to 2005). He became President-elect of the Neuromodulation Society of UK & Ireland 2005. He became President of the Neuromodulation Society of UK & Ireland from October 2006. In November 2008, he will step down as President but will continue on the board as immediate past president of the society. Dr. Valentine was chairman of the Anglian Pain Society between 1999 & 2005). Dr. Valentine was a member of the British Pain Society “Spinal Cord Stimulation for the Management of Pain: Recommendations for Best Clinical Practice” committee (2003-5). Dr. Valentine was the Pain Management training representative for the Anglian Region (1999 to 2002). Dr. Valentine was „Lead Consultant for Pain Management‟ at the Norfolk & Norwich University Hospital between 2000 & 2002. Dr. Valentine was „Vice-chairman of the Norwich Anaesthetics Department‟ during 2001 & 2002.

Roles within specialist societies:
The British Pain Society: Dr. Valentine sat on the British Pain Society (BPS) sub-committee set up to establish a national “best practice” document for spinal cord stimulation and the document “Spinal Cord Stimulation for the Management of Pain: Recommendations for Best Clinical Practice” was published in March 2005. This document can be accessed on the BPS website (www.britishpainsociety.org).

Neuromodulation Society of UK & Ireland: Dr. Valentine is a founding member and currently president of the Neuromodulation Society of Great Britain & Ireland (NSUKI). Further information is available on the International Neuromodulation Society (INS) website (www.neuromodulation.com).

Anglian Pain Society: Dr. Valentine was responsible for re-establishing the Anglian Pain Society in 1999 after a 5-year period of “dormancy”. He was the elected chairman of this multidisciplinary society until May 2005 when he handed over the responsibility to a newly elected colleague. During his years as chairman, the membership steadily grew and the Society continues to hold successful biannual meetings for all specialists involved in the treatment patients with chronic pain including doctors, nurses, psychologists, physiotherapists & occupational therapists.
