Directors’ Welcome Fort Sam Houston, Texas
Welcome to the Fort Sam Houston. Let us be the first to say “Thank You” for your service as a member of the United States Armed Forces. The Soldier and Family Assistance Center (SFAC) facility is located in the lower level of the Brooke Army Medical Center (BAMC) in Room L66-3b. We have a professional team established to assist you with your varied administrative and personal needs during your time as a Warrior in Transition (WT). This time will present many unique and some difficult challenges which the members of the SFAC team will work to assist you through. Keep in mind that although you may feel alone at times, you are not alone as we are here to help and assist you as much as possible. We also have a program called the Warrior and Family Support Center (WFSC) which has provided support and assistance to the WT and their Family Members since December 2003. This program is currently run in the Powless Guest House facility. Many of the Family Member questions and support requirements, as well as recreational activities, are met through this support program. The WFSC also provides “Fisher Foundation Hero Miles” which are available to all WTs and their Families to help with support and healing of the Warrior. As you progress through your recovery period, we are sure you will make use of this wonderful program which can provide respite from the daily routine in so many ways. Fort Sam Houston itself is known as the “Home of Army Medicine”, providing space and support to BAMC; which is a major medical treatment facility with medical research and education activities, as well as other medical training and operations commands supporting worldwide Army medical care for Soldiers and their Families. Fort Sam Houston is managed by the United States Army Garrison which supports the national defense missions of 65 tenant commands and activities. One of these includes the Installation Management Command (West Region). San Antonio is rich in history and culture. There are many opportunities to enjoy here in “military city, USA” along with the history and activities of Fort Sam Houston. The WFSC offers a wide variety of scheduled activities which include those in the San Antonio area. SFAC Mission Statement We sponsor mutual support, comradeship, and healing for Warriors in Transition and their Families by providing the best social, financial, educational, legal, vocational, and pastoral services available. We do so by creating a nurturing environment for these brave Warriors and their Families to gather. Our efforts are aimed at fostering dignity and independence, enabling the Warrior in their transition either back to duty or to service as a vital member of the community.

Soldier and Family Assistance Center (SFAC)
A one-stop location for support to Warriors in Transition (WT) and their Families
Fort Sam Houston has established a dedicated SFAC to provide tailored integrated support services for WT Service members and their Families. The SFAC, which also acts as an information clearing house, ensures these deserving Warriors and their Families receive the information and referrals they need in a timely and convenient manner. The SFAC’s goals are to:   Provide high quality customer service to WT Service members and Families; Provide/coordinate responsive and convenient services to WT Service members and Families at the SFAC or at a location convenient to the Service member or Family.  Provide a comfortable and safe place with appropriate resources to provide USO like services to WT Service members and their Families through the Warrior and Family Support Center.

Please note that SFAC services/assistance are not limited to the services listed above. The SFAC is a clearing house for information, referral and assistance. If you have a non-medical issue and not sure who can assist – start with the SFAC!

The SFAC is currently located in Brooke Army Medical Center (BAMC) on the lower level in room L66-3b. You can reach a SFAC representative at BAMC or by calling (210) 916-7322 between the hours of 0730 and 1630, Monday – Friday. Brooke Army Medical Center Brooke Army Medical Center is located at Fort Sam Houston on the northeast side of San Antonio, accessible from Interstate Loop 410, Interstate 35 and US 281. Brooke Army Medical Center is a state-of-the-art health care facility that provides level-one trauma care and graduate medical education. The multi-storied medical treatment facility is the primary structure in the medical complex. It has 1.5 million square feet of operational space and a bed capacity of 450. The Intensive Care Unit (ICU) has 48 beds and another 40 are dedicated to the Institute of Surgical Research Burn Unit. The hospital can expand to 653 beds if needed to support a wartime mobilization.

The hospital was designed with four distinctive structural elements of four subbuildings, with each letter of BAMC indicating a particular sub-building:  B (Bedtower Building) Located at the south end of the hospital. This subbuilding contains all the acute care wards and is the tallest component at seven stories above ground.

A (Ancillary Building) This sub-building has five floors above the lower level and contains the critical care units, radiology, laboratory and other support services. M (Medical Mall) This provides an open circulation area bridging between ancillary services and outpatient clinics. C (Clinics) This sub-building is located at the north end of the hospital. Its three floors contain most of the outpatient clinics. We’re here to support you!

The Warrior and Family Support Center (WFSC)
Since opening in Dec 2003 the WFSC has served 200,000 military personnel, family members and visitors. The WFSC offers unique support to the WT and their Family Members by offering a monthly calendar of trips and activities that include transportation to and from venues at no cost to the WT or Family Members. The WFSC provides a friendly, comfortable environment for the WT and their Family Members while they are in the care of BAMC staff. The WFSC averages 25 Special Activity programs a month. Through the Fisher Foundation, the WFSC has provided an average of 125 Airline Tickets a month to military Service Members and their Families through the Miles for Heroes Program with the Fisher House Foundation. The center is located on the second floor of the Powless Guest House and can be contacted at (210) 241-0811. Members of the U. S. Armed Forces do not have to be actively deployed to benefit from American Red Cross support. The Red Cross provides services to 1.4 million active duty military members and their Families.

How to access American Red Cross services  Active duty service members stationed in the United States and Family members residing in the service member's household (example: service member's spouse) should contact Armed Forces Emergency Service Centers for information and assistance 7 days a week, 24 hours a day, 365 days a year. Contact number: 1-877-272-7337  All Family members who do not reside in the service member's household, regardless of where the service member is assigned — at a local military installation or another geographical location — should contact their local American Red Cross chapter for assistance. Red Cross chapters are listed in local telephone directories and online at www.redcross.org, under "Find Your Local Red Cross."



Families living overseas may access assistance through the local on-base Red Cross office or through the base/installation operator for information if there is not a Red Cross representative on the local installation. 12

FREQUENTLY ASKED QUESTIONS AND ANSWERS
Q. How do the service members enter into Fort Sam Houston’s Warrior Transition Battalion (WTB)? Does anyone meet the OIF/OEF service members when they arrive? A.       Service members are admitted to the WTB via the following: Referral from the 3ID, Military Treatment Facility (MTF) Primary Care Manager and/or Case Manager during the mobilization or demobilization process Attachment of Service members on ADME orders Approval through Human Resources Command – Alexandria (HRC-A) for Service member to return on active duty for medical treatment via Medical Retention Processing 2 (MRP2) Service members permanent change to station (PCS’d) to undergo treatment or Medical Evaluation Board (MEB) / Physical Evaluation Board process (PEB) Reserve Component Service members receive Active Duty Medical Evaluation (ADME) Orders, attaching them to the Warrior Transition Battalion (WTB).

Yes, all OIF/OEF Service members arriving via MEDIVAC will be met by a Medical Department Activity Command (MEDDAC) staff member, who will escort service member(s) to the Brooke Army Medical Center Admissions and Dispositions (A&D) Office, then to the Emergency Room for medical evaluation and/or treatment. If necessary, the service member(s) will be admitted to the hospital for treatment; otherwise, the service member will be discharged with instructions to return on the next duty day. In all cases, an A&D Office representative will notify the service member’s unit of the service member’s arrival and request a representative to escort the Service member to his / her residence.

Q. How will Family members get to Fort Sam Houston’s Warrior Transition Battalion (WTB) and will someone meet them at the airport? A. Joint Federal Travel regulation (JFTR), Volume 1, chapter U5246, authorizes three relatives of a Seriously Ill / Very Seriously Ill (SI/VSI) patient to travel to and from the hospital at government expense when certain administrative requirements are met by the military medical treatment facility (MMTF) and the appropriate personnel or casualty affairs office. The patient’s physician uses SI/VSI criteria to decide who is eligible for Invitational Travel Orders (ITOs). (Eligibility for ITOs is based solely on medical criteria.)

ITOs for Family members of OIF/OEF patients will cover the cost of travel, lodging, and per diem for a pre-determined period of time. Typically, ITOs are not openended, although extensions are possible on a case-by-case basis. The Casualty Affairs Office, accessible through the Soldier and Family Assistance Center (SFAC), will be able to assist with extensions. The WTB will provide an Escort Officer who will meet the individual(s) traveling on ITO. Upon initial appointment, the Escort Officer will contact persons traveling on ITO to obtain additional travel information. The Escort Officer will ensure the persons being escorted are treated with professionalism, courtesy, and respect at all times. Upon ensuring that escorted persons have visited their Warrior; secured lodging and food, the Escort Officer will ask individual(s) to complete a Satisfaction Survey then conclude his/her duties. For Family members of patients who are not OIF/OEF, ITOs will only cover travel costs. The SFAC has no role in determining who is eligible and who receives ITOs. The SFAC staff is available to assist Family members with getting answers to questions related to ITOs. If there are no ITOs issued, there may be other avenues to obtain airline tickets. The non-profit Fisher House Foundation has teamed up with ―Operation Hero Miles‖ to provide eligible Service members undergoing treatment at a medical treatment facility incident to their service in Iraq, Afghanistan, or the surrounding areas with a complimentary, round-trip airline ticket. The tickets may be available to eligible Family and friends as well. The request form is available for pickup at the SFAC or Warrior and Family Support Center. The request must come from the patient. Ticket eligibility is determined by the Fisher House Foundation. There are multiple ticket restrictions. The Department of the Army provides an escort to all incoming Family members on ITOs. The Warrior Transition Battalion (WTB) is located at Building 3642 (located across the street from the Powless Guest House), on the Brooke Army Medical Center Campus, Fort Sam Houston, TX. Q. Where is the hospital located?

A. Brooke Army Medical Center (BAMC) is located at 3851 Roger Brooke Drive Fort Sam Houston, Texas 78234-6200. After 10 miles I-410 will merge with I-35 South. Travel for another 3.5 miles and take Exit 162, Brooke Army Medical Center/Binz-Englemann Rd. At the end of the exit ramp turn right and enter the George C. Beach Ave Gate. Coming from I-10/Houston (South East San Antonio) From I-10 North merge onto I-35 North. Travel for 10 miles and take Exit 162, Brooke Army Medical Center/Binz-Englemann Rd. At the end of the exit ramp turn right and enter the George C. Beach Ave Gate. Coming from San Antonio International Airport (North Central San Antonio) Exit Airport Blvd onto NE I-410 Loop. After 5 miles I-410 will merge with I-35 South. Travel for another 3.5 miles and take Exit 162, Brooke Army Medical Center/ BinzEnglemann Rd. At the end of the exit ramp turn right and enter the George C. Beach Ave Gate. Binz-Englemann Rd Gate Access This gate is open 24 hours and persons entering will only have access to BAMC. All occupants of vehicles (over the age of 16) are required to present current photo identification with an expiration date (example: military identification card, state driver’s license or state identification card). To obtain the pass you must show your drivers license and the reason you are requesting access to the installation. You may also be asked to show your motor vehicle registration and proof of insurance. All vehicles entering the installation are subject to search while on the installation. Concealed firearms are prohibited on this and any military installation. The speed limit when entering and exiting the installation is 10 miles per hour (mph).

Q. What documents do I need to bring with me? A.  Copy of all military orders  Military ID  Power of Attorney  Living Will  Immunization records for children in need of child care  Name and phone number of POC for the Active Duty patient’s parent unit  Valid Passport if returning OCONUS **Bring clothes for patient. Patients may only arrive in hospital pajamas and robes or may only have one military uniform. It is a good idea to pack a pair of sweat pants and shirt (can be cut for casts etc.), underwear, shoes/sneakers, and jacket / hat if weather is cold. Q. Is lodging available ? A. ARMY LODGING IS LOCATED ON THE BROOKE ARMY MEDICAL CENTER CAMPUS, FORT SAM HOUSTON POWLESS GUEST HOUSE Phone: (210) 357-2705 extension 2000 Location: Bldg. 529, George Beach Blvd. Powless Guest House is conveniently located on George Beach Boulevard near the Brooke Army Medical Center, off of Binz Engleman Road and Interstate 35. Powless has 150 rooms each with in-room telephone service, televisions, refrigerators and microwaves for the convenience of our guests. Please call for reservations. The cost is approximately $48 per night for one Family member. An additional $5 per night is added for each guest residing in the room. There is no charge for children 0-6 years old. Powless is a combination of suites and single rooms. A valid Military ID card is required to make reservations at Army Lodging. At Fort Sam Houston direct billing is available for Family members of Warriors in Transition traveling on Invitational Travel Orders (ITOs) at Army Lodging. If Army

Lodging cannot accommodate a Warrior or Family member, Army Lodging staff will refer the Warrior or Family member to a local hotel. Family members on ITOs will be able to submit off-post hotel receipts, up to the allowable government nightly rate, for reimbursement at the end of their travel. Keep your receipts!

Q. I do not have a Military Identification Card (ID). Do I need one, and how do I get one? A. Family members who have arrived at Fort Sam Houston Warrior Transition Battalion (WTB) and do not possess a military ID will be escorted to the SFAC to obtain one. Once you receive a military ID, you will have privileges at the Commissary and the Post Exchange (PX). The SFAC is located on the lower level of BAMC in room L663.

Q. How do OIF / OEF inpatients get a new military ID? A. Is the Warrior non-ambulatory? If yes, SFAC staff will contact the appropriate MILPO POC who will call the ward and make arrangements to bring in a portable ID machine to the patient’s bedside. Is the patient ambulatory, to include getting around in a wheelchair / walker / crutches? If the ambulatory patient has picture ID, he/she will be escorted to the ID card section at BAMC. If the ambulatory Warrior has no form of ID, he/she will need an E-7 or above / Platoon Sergeant to escort him to the BAMC ID card section. Q. Is transportation available from Fort Sam Houston’s Warrior Transition Battalion (WTB) to the airport? A. At Fort Sam Houston Family members traveling on ITOs will be met at the airport by their Escort Officer and transported to lodging. There is a USO office in the San Antonio International Airport to assist Family members with information. Family members traveling WITHOUT an ITO will be responsible for paying for the cost of transportation to and from Fort Sam Houston’s Warrior Transition Battalion (WTB). They will not be reimbursed for this cost.

Q. How do I get around once I have arrived? A. A shuttle service is available on the BAMC campus from the Powless Guest House to BAMC and to the Warrior Transition Battalion barracks. The best

transportation option would be a privately owned vehicle (POV). Car rentals are an option, but the cost is NOT reimbursable. Anyone with official business on post may use the on base shuttle buses. No ID is required. There is no shuttle bus service to off post sites. There are two shuttle bus routes on post; one is the BAMC or "Hospital Route" and the other is the "Brigade Route." The "Hospital Route" services most of the clinics on post, the Finance Office, the Military Police Station, and Army Community Service. The "Brigade Route" services the troop areas, Troop Dispensary, Dental Clinics, Military Clothing Sales Store and the Main Exchange. The routes are approximately 30-35 minutes. To obtain a copy of shuttle bus schedule, stop by the SFAC (located in the lower level of BAMC in room L663) or the Warrior and Family Support Center (second level of Powless Guest House).

Q. Is there parking available?

A. BAMC is a very large complex with large parking areas surrounding the facility. There is parking available at the Army Lodging as well.

Q. Where can I get something to eat or shop for items I need?

A. Meals are available at the following locations in the lower level of BAMC in the dining facility. Q. I have small children. Is childcare available?

A. Yes, childcare is available at the Child Development Center, located in Building 2530 and offers a developmental program for children between six weeks and five years of age. The standard hours of operation are Mon - Fri: 0530 – 1730, Preregistration is encouraged, although not required. Priority childcare is provided for Family members who want to visit their Warriors; have scheduled appointments, or who require respite care. If necessary, emergency childcare may be available. All Family members choosing to use the childcare services offered must provide the child(ren)’s immunization record. There is a Child and Youth Services representative in the SFAC (located in the lower level of BAMC in room L663). To register for Fort Sam Houston child care program, call (210) 221-4871 or DSN 471-4871 or call the SFAC Child and Youth Services representative at (210) 916-7322. School Age Services (SAS) The School Age Services Program is located in Building 1705 off Winans Road. The SAS Program offers a Before and After School Program, Open Recreation Program, Reserve Hourly Care, Seasonal Camps (Spring, Fall, Winter, and Summer) and Teacher In-Service Camps for children in first through sixth grades. The Before School Program operates Monday-Friday from 6 a.m. until school begins on Fort Sam Houston Elementary School regular school days. On and Off Post Lodging

Rooms for Warriors are provided by the Warrior Transition Unit. At Fort Sam Houston direct billing is available for Family Members of Warriors in Transition traveling on Invitational Travel Orders (ITOs) at Army Lodging. If Army Lodging cannot accommodate a Warrior or Family Member, Army Lodging staff will refer the Warrior or Family Member to a local hotel. Family Members on ITOs will be able to submit off-post hotel receipts, up to the allowable government nightly rate, for reimbursement at the end of their travel. Keep your receipts! Lodging for Warriors is based on the Service Member’s rank and medical condition. Billeting is available in barracks for Service Members in the rank of E1-E6. Billeting for Service Members E7 and above is provided in the Army Lodging. Service Members who require special accommodations will be billeted in other than the barracks.

Government Military Housing (GMH)
Warriors and their Families will have priority for on-post housing, administrated by the Garrison through Government Military Housing (GMH). Approval is granted through the Community Manager of GMH Housing.

Fort Sam Houston Army Lodging
Army Lodging offers double rooms, single rooms and Distinguished Visitors quarters. The Powless Guest House is conveniently located on George Beach Boulevard near Brooke Army Medical Center, off of Binz Engleman Road and Interstate 35. Powless has 150 rooms, each with in-room telephone service, televisions, refrigerators and microwaves for the convenience of our guests. Please call for reservations (210) 357-2705 extension 2000.

History Fort Sam Houston was founded in 1845, and named for General Sam Houston, hero of the battle of San Jacinto and first president of the Republic of Texas. Fort Sam Houston is unique among Army posts. On one hand, it is a major, active military installation and plays a vital role in the defense of the United States. On the other hand, it contains some of the oldest structures on any Army installation. Two of those sights are the Pershing House and The Quadrangle.
