October 1, 2009 Dear Parents and Guardians, This notification is being sent to you in conformance with the Children’s & Families Protection Act (Chapter 85 of the Acts of 2000). The Act requires the Church to notify you when pesticides are applied on the outside of public and private schools, day care centers and school age child care programs. Marshfield United Methodist Church, 185 Plain Street, Marshfield is having this location treated with the Sentricon Termite Baiting System in response to the presence of termites. The Church selected this treatment method to reduce risk and hazard to our student population and staff. At various times during the course of the year, Waltham Services, Inc. may remove the monitoring devices placed below ground in plastic holding stations and replace them with baits lethal to termites. The pesticidal bait is non-volatile and not accessible or visible from above. Please access the Church web site for additional information @ www. Harbormarsh.org. Click on Resources, then IPM, which will provide you the Consumer Information Bulletin, Department Standard Written Notification Form, and Chemical Fact Sheet. We are currently using Option C of the Standard Written Notification Requirements. If you do not have access to online information, hard copies of above mentioned information are available in the Church office. Please contact Barbara Freeman at 781-837-8010 should you have questions, would like more information. Barbara Freeman Director MUMC Pudding Hill Preschool
