Does the LHD work with employers to increase opportunities for physical activity and healthy eating for employees? 7. Does the LHD work with other stakeholders in the community to increase opportunities for physical activity, heart healthy food choices, and improved medical treatment for CVD. 8. Does the LHD conduct physical activity or healthy eating campaigns (e.g., walking, low-fat milk or fruit and vegetables) in the community? 9. Does the LHD participate in local land use and transportation planning activities (comprehensive plans, site plan reviews, etc.) to assure the availability of adequate facilities for bicycling, walking and recreation?

Does the LHD incorporate evidence-based practices for reducing CVD risk factors in the community and clinical guidelines to prevent and/or treat CVD in services provided by the LHD and/or provide guidance to local health care providers on evidence-based best practices for reducing CVD among their patients? 12. Does the LHD work with stakeholders to implement one or more strategies in the New York State plan for cardiovascular health? 13.
