Change Order
This is a Change Order to the Name of Original Agreement between Parties (ie. Building Contract) dated Date of Original Contract (the "Original Contract") between Name of Contractor (as in Original Contract) (the "Contractor") and Name of Principal (as in Original Contract) (the "Principal"). 1. The parties hereby agree that the following change(s) (the "Changes") shall be made to the services to be provided by the Contractor in the Original Contract: Insert brief description of change to work and services to be provided by Contractor

As consideration for the Changes, the Principal shall pay to the Contractor the additional sum of Cost of Requested Change (ie. $500.00), which shall be paid Specify when Payment of Change will be made (ie. concurrently with the signing of this Change Order, or upon completion of the work or services to be performed under the Original Contract, or on July 31, 2001) The parties hereby confirm that all other terms and conditions of the Original Contract are in full force and effect, unamended except as expressly provided in this Change Order or in any other Change Order agreed to in writing between the parties hereto.
