1. IMPORTANT NOTICE
1.1. This Pre-Qualification Questionnaire is only to be used by those registered with NHS
PROCUREMENT NORTH EAST Commercial Services Division as expressing an interest in
the supply of IAPT Services, their professional advisers and other parties essential to
preparing a response to this documentation and for no other purpose.
1.2. All documentation of whatsoever nature provided to you for the above purpose
shall remain the property of NHS PROCUREMENT NORTH EAST, shall be made
available on the basis that it and the information it contains are treated in the
strictest confidence and must be returned on demand.
1.3. This Questionnaire is made available in good faith. No warranty is given as to the
accuracy or completeness of the information contained and any liability for any
inaccuracy or incompleteness is therefore expressly disclaimed.
1.4. NHS PROCUREMENT NORTH EAST reserves the right to cancel the tendering process
at any point and is not liable for any costs resulting from any cancellation of this
tendering process, nor for any other costs incurred by those expressing interest in or

commissioning guidelines and European and UK legislation, particularly in relation to

2. TRUSTS INVOLVED

3. CONTRACT NEEDS & LOCATION
3.1. NHS County Durham PCT & NHS Darlington PCT
3.2. seeks to commission Services that will deliver high quality Psychological Therapy
Services fulfilling the requirements of the Department of Health Improving Access
to Psychological Therapy (IAPT). The Services will be provided across the Durham
and Darlington locality.
3.3.These new Services will work with existing Providers, Commissioners and local
communities to fully implement National and Local IAPT requirements. This will
include increasing the number of appropriately qualified practitioners to deliver
Psychological Therapy in conjunction with commissioned Education Providers,
improving access through better waiting times and implementing Stepped Care
models for anxiety and depression. Key objectives will also include improving
vocational outcomes and employment opportunities for people experiencing
anxiety and depression and for providers to be a key partner in developing a whole
system approach for mental health and wellbeing.

4. TIMETABLE & PROCEDURE
4.1. The responses to the questions in this PQQ will be used to draw up a shortlist of
suppliers who will then be invited to tender for the services outlined. A list of the
main activities and timelines involved is listed below; please keep these in mind as
they are fixed for the project and failure to meet any one of these elements may
result in a disqualification from the entire tender.

6. INSTRUCTIONS FOR COMPLETING THIS DOCUMENT
6.1. Please ensure that you complete all sections and feel free to supply additional
information on separate sheets where there is insufficient space on the questionnaire
itself. Non-completion of any sections may render your organisation ineligible to bid.
6.2. Please ensure that any supporting documentation is clearly marked with your
organisation’s name and is cross referenced to the appropriate section in the
questionnaire. Do not include attachments or other supporting documentation in the
body of the PQQ – a separate attachment is required.
6.3. Do not provide evidence or supporting documentation as web links or as references
to documents not supplied – you must supply the required information along with the
completed questionnaire.
6.4. Where reference to large documents is necessary please provide the relevant
extract, if possible, rather than the whole document.
6.5. Where possible, submissions, and any associated documents should be presented in
Portable Document Format (PDF) in order to minimise any potential compatibility
issues.
6.6. Answer each of the questions raised for the organisation expressing an interest in
the project and for each business if the application is a consortium, joint venture or
subject to any other joint arrangement.
6.7. PQQ submissions that are received late, i.e. after the published deadline, are
incomplete, have errors or are confusing in any way may not be considered further.
