This form is printed on a light blue (pastel) colored paper. You know---the
“B” in Blue as the “B” in Benefactor.

Color coding these forms is very important. One will realize this when after
a successful presentation people are asking to “sign up”. It was after such an
event that I devised these forms.

There were almost 100 people in the room and some wanted to be
Sustaining, Paul Harris, Benefactors and/or Bequest Society Members. I
was trying to use the Rotary Foundation generated forms and there I was
trying to find the correct form for each of the members. It was a great
success, yet a total disaster. Success in that we signed up more people than
had ever participated. Disaster in that only about half of the people who
asked to sign up, actually received the right form.

Now, when someone wants to sign up for a specific Foundation program, all
one has to do is glance down and by color select the proper form. One
hardly has to lose eye contact. Also, all of the information required by The
Rotary Foundation is on the form PLUS there is credit card information on
every form.

The Rotary Foundation will accept these forms. Therefore the information
does not have to be transcribed on to their forms. Their only request is that
they prefer to have the forms sent in as a package rather than one at a time.
Of course, this goes for all of the forms, even their own. It is easier for them
to process several at a time from one Club rather than getting them one at a
time.

Finally, note that the “Share-DDF” has been pre-checked on this form. We
found that before we did this the members expressed frustration, not
knowing which one to choose. Since almost all of our members checked
“Share-DDF” after it was explained to them, we decided to just pre-check it
on all forms. If this is not the case we just make the necessary change for
them.
