 SAMPLE PROGRAMME AND PROVIDER EVALUATION BY LEARNERS
As a final year learner, you will receive an e-mail via your Institutions from PAB
during August 2006. PAB invites you to complete the survey on the form attached.
You will be asked for some personal data in order to verify that you are eligible to
take part and is registered on our database as a South African learner towards a NQF
or accredited qualification. The survey is simple and should take no more than five
minutes to complete. Please ask your Institution to fax it back to us on (012) 3096034.
