GENERAL SURGICAL EXPERIENCE
I trained in General Surgery, Orthopaedic Surgery, Neurosurgery and Urology over a period of almost
four years, during which I kept a record of the procedures performed without supervision. It includes
15 amputations, 50 appendicectomies, 13 bowel resections, 95 inguinal hernia repairs, 42
circumcisions, 70 procedures for varicose veins, 98 cystoscopies and resections of bladder tumour, 11
open fracture reductions and 27 orchidopexy and orchidectomy procedures. I supervised fracture
clinics, and looked after patients in Intensive Care.


PLASTIC SURGERY EXPERIENCE
As a Trainee
After training in General Surgery, I trained in Plastic Surgery over a period of eight years, working at
five different units in the United Kingdom. I also spent six months working at the Hospital for Sick
Children in Paris. During this time I assisted at a wide variety of procedures including craniofacial
surgery, cleft lip and palate surgery and free tissue transfer. Before becoming a consultant, I
personally performed 2636 procedures. This includes 1515 procedures in General Plastic Surgery and
Burns, 667 Hand Surgery cases, 321 Head and Neck cases (including cleft lip and palate), and 133
Cosmetic Surgery procedures.

At St. Thomas's Hospital and the Hospital for Sick Children, Great Ormond Street I worked with
Epidermolysis Bullosa patients on whom I performed procedures to release finger contractures and
syndactyly. St. Thomas's Hospital had an Argon laser which I used to treat patients with port wine
stains with good results.

During my six months working in the Head and Neck Unit at the Royal Marsden Hospital, I worked with
the reconstructive team to resect intraoral and facial tumours and to reconstruct the defects with
cutaneous, myocutaneous and free flaps.

I have a special interest in the correction of ear deformities, and set up the first units in the UK, at
Great Ormond Street Hospital, London, and at Mount Vernon Hospital, Northwood.


As a Consultant
I have maintained my interest in ear reconstruction, and have to date reconstructed 700 ears with a
carved costal cartilage framework. Many of these cases come from abroad. I also provide bone
anchored prosthetic ears for patients who prefer this technique. Between December 1992 and
October 2006, I ran Laser Services at Mount Vernon Hospital, treating a variety of vascular lesions,
pigmented lesions and tattoos. In conjunction with the Department of Medical Optoelectronics at the
Swansea Institute, I developed a laser for depilation and undertake hair removal clinics.

TEACHING
As a trainee I regularly lectured to student nurses and medical undergraduates. As a consultant I now
teach on a variety of courses from Minor Surgical Techniques for General Practitioners to ear
reconstruction and laser treatment for specialist plastic surgeons.

RESEARCH
Prompted by cases of chemotherapy extravasation referred with extensive tissue loss and scarring
around nerves and tendons, I studied children who suffered soft tissue loss after drug extravasation
injuries. In some instances, despite extensive reconstruction, hand function remained severely
compromised. Early removal of noxious materials (calcium, potassium, adriamycin and daunorubicin)
from the subcutaneous perivenous space by liposuction or saline flushout techniques has proved most
rewarding. One hundred and ten such patients have been studied, and the results of this technique
are quite striking.

During my French Exchange Fellowship I developed a method of calculating intracranial volume from
CT scans, which was then used to study 100 children with craniosynostosis. Contrary to traditional
teaching, it was found that the majority have a normal intracranial volume.

Neonates with misshapen ears can have the deformity corrected with a simple splint if this is applied
at birth. The floppy cartilage moulds readily and gives good long term results. In most cases this
avoids the need for surgical correction later. This work was presented to the British Association of
Plastic Surgeons and received widespread media coverage in December l994. As a result, I
developed the Ear Buddy, a device now available worldwide for the treatment of new-born infants with
ear deformities.

A review of the psychological benefits of microtia reconstruction has been undertaken. As a
refinement to the surgical technique, those patients undergoing autogenous tissue reconstruction of
ears now have the ribcage reconstructed with unwanted fragments of carved chest cartilage and the
vestigial ear remnant. This development appears to reduce pain in the short term and may reduce the
risk of chest wall deformity in the long term.

At the Hospital for Sick Children, Great Ormond Street, I undertook a joint microtia clinic with the Ear,
Nose and Throat Department and the Department of Medical Genetics, and began several projects to
examine the possible causes of significant congenital ear anomalies.

In a combined project with the Swansea Institute a laser to remove hair was developed. The
technology is now in use world-wide. It has a variety of clinical applications and clinical trials have
shown it to be successful in removing hair for long periods of time. Further research to improve
results is underway.

As a result of collaboration with Gyrus Medical and Rhytec, a nitrogen-driven plasma kinetic device for
skin resurfacing was developed. This passed into commercial use in the US in 2006 and was
introduced into the UK in 2007.
