Introduction
To provide special recognition for the service of military veterans of World War II, the Korean War, or the
Vietnam War, Utah Code Ann. § 53A-3-427 authorizes the Weber Board of Education to award honorary
high school diplomas to veterans of these wars who left high school before graduating to serve in the armed
forces of the United States. The Weber Board of Education recognizes the important contribution and
sacrifice these generations of men and women made in our nation’s defense in awarding this honor.

Eligibility
To be eligible for an Honorary High School Diploma, the veteran must meet all of the following
requirements:
1.       Before graduating from high school, the veteran enlisted in or was drafted into the armed forces of
         the United States during the period of World War II, the Korean War, or the Vietnam War.
2.       Veteran was honorably discharged or was released from active duty because of a service-related
         disability.
3.       Veteran resides within the school district or resided within the school district at the time of leaving
         high school to serve in the armed forces of the United States.
4.       Veteran did not subsequently graduate from high school.

Application Procedure
1.     Complete application form. The application form may be completed by the veteran or an immediate
       family member on the veteran’s behalf if he/she is deceased or incapacitated.
2.     Provide a copy of the veteran’s discharge/separation from military service indicating honorable
       service during the period of World War II, the Korean War, or the Vietnam War.
3.     Applicant’s signature certifying information provided.
4.     Completed application should be returned to the Weber District Secondary Education Department.
5.     The school district may verify the dates of the veteran’s school enrollment from their own records.

Replacement of Discharge Papers from Military Service
A veteran or next of kin may request a copy of the veteran’s discharge from military service from the
National Records Center, Military Personnel Records, 9700 Page Blvd., St. Louis, MO 63132-5100. All
requests must be in writing; the National Records Center recommends submitting the request using Standard
Form 180 “Request Pertaining to Military Records.” You may print the form at
http://www.archives.gov/research/order/standard-form-180.pdf. You may also be able to obtain the ST-180
from Federal Information Centers, from local Veterans Administration offices, or from veterans service
organizations.

Requests require the signature of the veteran or next of kin, if the veteran is deceased or incapacitated.
Please include the veteran’s full name while in the service, date of birth, branch of military service, service
number or social security number, and approximate dates of military service. If the request pertains to a
record that may have been involved in the 1973 fire, also include: place of discharge, last unit of assignment
and place of entry into the service, if known. The workload of the National Records Center may not permit a
prompt response to a request.

This application is used to establish the eligibility of an honorable discharged veteran for receipt of an honorary high
school diploma from the Weber Board of Education, Ogden, Utah, in accordance with Utah State Code Ann. § 53A-3-
427. The veteran must have actively served in the United States armed services during the period of World War II, the
Korean War, or the Vietnam War.

      This completed application is to be returned to the Weber District Secondary Education Department.
