CAMELBACK SIGN UP FORM – DUE TUESDAY, November 20th at 2:30
I wish to be signed up for the Camelback Friday ski trips that are to run on January 8th, 15th, 22nd,
29th and February 5th 20010. (No school time will be missed for this trip.)

The trip will leave from Voorhees at 2:45 p.m., and will return at 9:15 p.m. each Friday night. Students
should be at the school by 7:00 am on Friday for equipment drop off. Please bring equipment to athletic
entrance of the school. Students must ride the bus up and back with Voorhees and cannot meet the group at
the mountain.

       PLEASE READ ALL INFORMATION CAREFULLY

                       GENERAL INFORMATION
 Students must ride the bus transportation to and from Voorhees High
  School. Students will not be allowed to meet the group at the mountain or to
  leave with another person. Failure to comply with these rules will forfeit
  student’s ability to participate in trips without refund.
 Refunds will only be issued with a doctor’s excuse. The written doctor’s excuse
  must be given to the group leader. Be sure to include group name, parent/guardian
  name, participant’s name, home address, phone number and what was purchased
  for the program. Money for bus transportation is not refundable.
 When your child misses a scheduled group trip, the students pre-paid ID card will
  have an extra pass on it. VHS’s ID will be valid for any Monday-Friday for the
  session that their group skis, be it day, twilight or night. If your child has
  purchased rentals in this program, their rental will also be on the voucher.

                     SKI SCHOOL INFORMATION
   Lesson times will be assigned once Camelback has received participant’s
    information. They will try to schedule lessons at a convenient time based on
    arrival. It is very important that group leaders stress the importance of being on
    time to all group members.
   Camelback will make every effort to keep members of your group together in
    classes but due to the volume of children on certain nights, we may need to
    combine children from different groups in ski/snowboard school lessons.
   If more than one lesson is missed, all remaining lessons are forfeited.
   No substitutions are permitted for absentee lessons.
   Due to the amount of testing that must take place on the first visit we ask
    everyone to be patient and understanding.

                          RENTAL EQUIPMENT
   No one other than the person who purchased the equipment can use the rental
    forms.
   Snowboard rentals are only available midweek/non-holiday. If a snowboard is lost
    or stolen, the person whose name is on the form will be charged. They will not be
    able to participate in the program until the charge has been paid. If wristguards
    are checked off on the snowboard rental form, the renter must take them each
    week.

                             CANCELLATIONS
   If Voorhees cancels school, the group’s visit that day will also be cancelled. If
    Camelback must cancel groups due to weather conditions on the mountain or for
    any other reason, we will contact the group leaders by 11:00 am. Make-up visits
    may not always be possible.
            SKI CLUB POLICIES
     1. HELMETS are MANDATORY and MUST BE WORN AT ALL
TIMES!!! WRISTGUARDS for SNOWBOARDERS are MANDATORY
and must be worn at all times!

     Potential consequences if caught without a helmet on:
     1st offense: Ticket will be revoked for the remainder of
     the evening. NO REFUNDS OR CREDIT WILL BE ISSUED!!!
     2nd offense: Termination from the VHS Ski Club for the
     season and forfeiture of any and all costs incurred.
     NO REFUNDS OR CREDITS WILL BE ISSUED!!!

     2. Any reports forwarded to VHS Ski Club from
        Camelback personnel or Ski Patrol for
        insubordination, reckless skiing, etc…(Rule of
        thumb; if it is wrong in school – it is wrong
        during the ski trip)
     Possible consequences:
         a. Automatic termination from VHS Ski Club
         b. Automatic Referral to Disciplinarian
         c. NO REFUNDS OR CREDITS WILL BE ISSUED!!!

     3. Chaperones can be contacted by asking any ski patrol or
ranger member. Head chaperone carries a pager provided by
Camelback mountain.

     4. Always ski with a companion!!!

     5. Report all accidents to ski patrol, lift
        attendant, chaperone or another person who
        can help. All injuries must be reported to
        Miss Luckhardt or Mr. Hunt.
