         2009 Partnership Middle/Junior High School Honor Choir Application
                PLEASE READ CAREULLY AS THIS FORM INCLUDES UPDATED INFO!
♫♫ Each participating sponsor/school may send up to THREE balanced TRIOS at $120 per
   trio. Remember, sponsors must have active CMEA Membership. Checks are made
   payable to CMEA/SBS Honor Choir.
♫♫ Communication will be done through you, the choir director and/or sponsor. If you expect
   to be unavailable during the rehearsal or concert, you must provide a contact person.
   Your presence at the rehearsal is greatly appreciated.
♫♫ Included in the fee, each student will receive his or her own octavos. However, if
   teachers or voice coaches would like their own personal octavos, please include an
   additional $20.00.

                 2009 Middle/Junior High School Honor Choir Participants
Dear Parent or Guardian of_____________________                       ,

Congratulations! Your student has been invited to participate in the Honor Choir 2009 by his or her
music teacher. This is a wonderful opportunity for each student as they experience advanced
standards in choral singing and share in an always memorable and rewarding event! As the child’s
parent or guardian, please read the following information carefully as it contains important information
concerning participation and performance with the Honor Choir.

FEES: Your student’s fee is_____________. This fee includes original copies of music, a rehearsal
CD, guest director/accompanist, venue fee, honor choir pin and 2 free tickets to the concert.

REHEARSAL POLICIES/INFO: IMPORTANT!
  ♪ Festival Choir members must attend the full date of rehearsal to participate in the concert.
  ♪ There will be a full day of rehearsals beginning at 9:00 a.m. at Pt. Loma Nazarene University
     (3900 Lomaland Dr., 92106), Saturday, March 7, 2009.
  ♪ Students are expected to be on time and in their seat ready to rehearse. Transportation is the
     responsibility of the student/family. Students are required to be picked up on time, as there is
     no supervision for them after the concert.

BEHAVIOR POLICY/EXPECTATIONS: Each singer is expected to behave courteously to other
singers and adults. They are expected to have their music learned prior to the first rehearsal, act
professionally, exhibit good musicianship skills and stay on task during the entire rehearsal. A singer
who does not exhibit the above WILL NOT be allowed to perform in the final concert and their school
will not be allowed to send students the following year to HONOR CHOIR.

PERFORMANCE DETAILS:
   ♪ The concert is Saturday, March 7th at Point Loma Nazarene University (3900 Lomaland Dr.,
     92106) in Brown Chapel at 7:00 pm at. Parking is available in the structure just south of the
     chapel.
   ♪ This is a dressy event and singers are to bring their concert clothes from their school choir or
     other appropriate performance outfit if the school doesn’t have a uniform, unless the director
     informs otherwise to change into for the concert.
   ♪ Doors will open at 6:30 PM. Audience members should line up outside with their ticket.
   ♪ Students will each receive two free tickets for family members. Additional tickets are $2.00
     each and will be available at the door.

Please keep this sheet handy for reference! Make sure you have signed the parent permission slip
and as always, thank you for supporting such talented and musical individuals!

We understand the transportation, attendance and behavior policy (listed on the information
letter) and that the participating student named above may be asked to leave the Honor Choir
if behavior or attendance becomes a problem. We understand the fee schedule and that
there are NO REFUNDS.

California law (education Code 1081.5) provides that any person making a field trip or
excursion waives all claims against the school district and the State of California for injury,
accident, illness, or death occurring during or by reason of the field trip or excursion.
ACCORDINGLY, I/WE HERBY WAIVE ALL CLAIMS WHICH I/WE MIGHT HAVE AGAINST
THE SCHOOL, DISTRICT, and THE STATE OF CALIFORNIA, their officer, agents, and
employees for injury, accident, illness, or death occurring during or by reason of the above
described activity.

I also authorize the organization to use my voice and likeness in the media for promotion of
this event (or the voice and likeness of the above stated minor of whom I am the legal
guardian). Furthermore, there is to be no charge on my part (or on behalf of named minor, if
applicable) and it is understood that no fee has been authorized for my cooperation of the
above named minor.
