STEP 1. Qualifications For Electronic Filing
1. Your tax return must be LEGIBLE, and FOR THE CURRENT TAX YEAR ONLY. 2. Social Security Numbers 900-00-0000 through 999-99-9999 are not allowed for electronic filing. Name, Date of Birth and Social Security Numbers for all persons on the tax return must match exactly with their most recent Social Security card. Birth dates for all persons on the tax return must be listed on the Payment Authorization Form, Section II. 3. The IRS does not allow electronic filing with the filing status of “Married Filing Separate” if you live in Arizona, California, Idaho, Louisiana, Nevada, New Mexico, Texas, Washington, or Wisconsin. 4. You must have an account at the financial institution from which you received this packet to use this service.
STEP 2. Complete and sign Form 8453
1. Complete the heading with name(s), social security number(s) and address. 2. Complete Part I, Tax Return Information lines 1 – 5. IMPORTANT: Use the appropriate line numbers denoted on the 8453 according to the tax form you are submitting. 3. Complete Part II, Declaration of Taxpayer. You must check box 6a to receive a direct deposit. 4. Sign and date Form 8453. If filing a Joint return, your spouse must also sign and date the form.
STEP 3. Complete and sign the Payment Authorization Form
COMPLETE ALL SHADED PORTIONS OF THIS FORM. We must have a phone number and/or an e-mail address to be able to contact you if we have a question regarding your return. If you fail to provide us with a current working phone number and/or e-mail address, your refund could be seriously delayed. The account designated to receive the direct deposit must be in the taxpayer’s name.
STEP 4. Items to include in the tax packet
1. Completed, legible federal tax return and state return (if applicable). It is the responsibility of the taxpayer to file a complete and accurate return. 2. The original signed Form 8453 completed and state electronic filing form (if applicable). 3. The original Payment Authorization Form. 4. All W-2’s, 1099’s, and SSA forms. This includes 1099-R, 1099-G, 1099-MISC and W-2G as well as any other forms or schedules necessary to complete your return. 5. Please keep copies of all information enclosed in the tax packet. TRE will not furnish any copies.
STEP 5. Check the status of your tax return
1. Call our 24-hour automated status line at 1-800-706-0902 three business days after submitting your return. 2. It is the taxpayer’s responsibility to check the status of his or her return using the automated tax return status line at 1-800-706-0902. Tax Refund Express will not be held responsible for any delays. 3. Any tax returns with an error requiring any additional forms to be sent to Tax Refund Express will be assessed a one-time $15.00 fee. 4. REJECTED TAX RETURNS: If your tax return is rejected, please call TRE Customer Support at 1-800-934-4904 to verify the reason for rejection. After additional information is received, TRE will re-submit your tax return immediately to the IRS. (There are NO additional charges for rejection re-submission unless additional paperwork is required.) STATE TAX RETURNS: If your financial institution offers state electronic filing, you must submit your state tax return together with your federal return. You cannot electronically file your state return without the federal return. State Revenue offices have independent timetables and methods for dispersing their state refunds.

In the event, TRE, or the financial institution named above fails to perform in a manner substantially consistent with the aforementioned service guarantee, customer’s sole and executive remedy is limited to a refund of the processing fee. Tax Preparation Disclosure Statement
Tax Refund Express, Inc. (TRE) is the sole provider of the preparation service hereby offered. Employees of TRE, not of the financial institution, are responsible for the preparation of all tax returns. TRE will contact each taxpayer(s) regarding his/her return, provide a copy of said return to the taxpayer(s), and obtain all forms necessary for the electronic filing of the tax return. TRE will not be held responsible for errors due to taxpayer(s) failure to provide TRE with all necessary documentation in order to prepare a complete and accurate return.

All shaded areas on the Payment Authorization Form must be completely filled out. The Child Tax Credit can only be taken for a child who was born between January 1, 1987 and December 31, 2003. If you received a check from IRS in 2003 for the Advance Child Tax Credit, you must indicate the amount received in the shaded space immediately below the dependents section on the Payment Authorization Form. When filing electronically, taxpayers must provide a copy of all W-2’s, W-2G’s, and 1099 Forms (1099R, 1099G, 1099M) to Tax Refund Express in order to file your return.
