					PEDIATRIC REGISTERED NURSE PERFORMANCE BASED WORK STATEMENT IN ADDITION TO THE BASIC CONTRACT, THE FOLLOWING EXTENDED DESCRIPTION APPLIES. 1. 1.1. GENERAL. DESCRIPTION OF WORK.

1.1.1. The contractor shall provide the services of Pediatric Registered Nurses to include phone advice in the Pediatric Outpatient Clinic, Department of Pediatrics, at (Location). 1.1.2. This is a personal services contract and is intended to create an employer-employee relationship between the Government and the individual Health Care Provider (HCP). The performance of the individual HCP under this contract is subject to day-today supervision, clinical oversight and control by healthcare facility personnel comparable to that exercised over military and civil service HCPs engaged in comparable work. 1.1.3. Performance Measures (Sample)

1.1.3.1. Outcome. The contractor will provide qualified personnel who will fulfill the requirement to provide Pediatric Registered Nurse services at a standard enabling the MTF to provide such services at a high level of quality. 1.1.3.2. Standards. Acceptable measures include: fill rate95%; employee turnover rate-30% per year; substantiated patient complaints-max 2 per year per provider. Other performance evaluation factors will be monitored that are not quantified by numerical measurements which include: contractor providing personnel exceeding the minimum qualification standards; patient customer service comments; provider and contractor relationship with hospital staff/government contracting personnel; compliance with hospital policy and procedures; timely patient follow-up when necessary. 1.1.3.3. How Measured. Department/clinic supervisory personnel will monitor provider performance through government information systems and records, patient records, customer service information, contractor reports, and time sheets. 1.2. QUALIFICATION. Qualifications: In addition to the qualifications given here, the nurse shall have sufficient experience to perform the tasks and procedures identified in this work statement. 1.2.1. Have and maintain current certification in cardiopulmonary resuscitation (CPR).

1.2.2. Have a current license to practice nursing in at least one state in the United States. 1.2.3. Be a Certified Registered Nurse.

1.2.4. Be a graduate of an approved school of nursing and hold current registration as a registered nurse. 1.2.5. Possess evidence of continuing nursing education.

1.2.6. Have at least one year of experience in the pediatric outpatient environment. 1.4. HOURS OF PERFORMANCE. The contractor shall provide one Pediatric Registered Nurse Monday-Friday 1530-1930 hrs and Saturday-Sunday 1000-1530 hrs 52 weeks a year, excluding Christmas Day and New Years Day. 5. SPECIFIC TASKS. following tasks: The contract nurse shall perform the

5.1. Full range of nursing services with the primary responsibility being to serve as the point of contact for all matters relating to the Pediatric Advice Line. The Pediatric Advice Line provides direct parental access to the pediatric outpatient clinic for general information regarding pediatric routine, minor illnesses, symptoms, complaints, and acute minor illnesses. 5.2. Performs telephone triage by obtaining pertinent history and makes assessment to determine if the patient should be examined immediately, in the near future, or not at all. Educates parents regarding treatment modalities, and prescribes treatment plans in accordance with established pediatric advice line protocols. Maintains written records of all telephone contacts. Becomes familiar with over-the-counter medications for pediatric minor illnesses. Maintains good rapport and collaborates with staff pediatricians, nurses, and nurse practitioners when circumstances surrounding calls are beyond experience of advice nurse. Returns calls as needed. 5.3. Schedules appointments as needed.

5.4. Becomes familiar with department and clinic standard operating procedures (SOPs) concerning immunizations, appointments etc.
