Section A - Personal Data- Complete in full NOTE: Canadian citizens born outside Canada or currently residing outside Canada and those who have completed studies outside Canada must provide proof of citizenship (e.g. photocopy of birth certificate. Permanent residents (landed immigrants) must provide a photocopy of their Record of Landing form as proof of immigration. International applicants studying on a Minister's Permit must provide a photocopy of the permit and the Approval in Principle letter, if applicable.
Section B - Application Fees - Complete in full A non-refundable fee must accompany all applications and re-applications in the form of a cheque or money order (no cash or post-dated cheques) made payable to "The University of British Columbia".
Applications will not be processed unless the appropriate application fee is received (before the deadline). NSF cheques will be charged an additional $25.00. Canadian Citizens and Permanent Residents International/visa applicants on a student authorization $ 60.00 $100.00
Section C - School of Social Work Equity Policy - Complete if applicable. The School has a self-identifying equity policy affecting the Aboriginal Peoples of Canada, ethnic minorities, and the disabled. A 5% weighting will be awarded to those applicants meeting criteria. If you wish to be considered under this policy, please check the box and complete the Application Supplement III. Section D - Re-application to the BSW If you have, in previous years sent in an application to the BSW Program, please indicate this. (Reapplication does not affect your evaluation).
Section E - Part-time or Full-time Please indicate your choice of full-time or part-time studies. Section F – Previous studies at UBC. If you have studied at UBC please indicate your Student Number. Section G - Academic History - Complete Section G in full and arrange for two copies of all transcripts to be included in your application package. Only show degrees that have been conferred, or, if in the near future, the date of convocation. Section H - English Language Test Results - Complete only if applicable English Language Admission Standard - Applicants whose first language is not English are required to demonstrate competence in the English language prior to admission. A Test of English as a Foreign Language (TOEFL) score of 580 (237) or better sent directly from the testing agency would satisfy this requirement. See UBC Calendar for exemptions (http://students.ubc.ca/calendar). Section I - Declaration of Applicant Applicants must read and sign the Declaration. NOTE: Due to limited enrolment and competitive admission to the School of Social Work, applicants are advised to consider applying to more than one university program in Social Work. Applicants to social work wishing to be considered, as a second choice, for admission to another faculty or major at UBC must complete a separate Undergraduate Admission Application Form from the UBC Office of the Registrar, Admissions Office, Brock Hall. Applicants wishing to be considered for the UBC Social Work program only are not required to complete a separate application to the University.
Please enclose a stamped, self-addressed postcard of your choice with your application for the School. We will put this card in the mail to you to acknowledge receipt of your application package.
If you are still taking courses, your grade point average (gpa) will be calculated as at December 31 st, so it is important to provide us with transcripts up until that date. Applicants must submit two (2) official transcripts from every post-secondary institution attended (photocopies and faxed copies not acceptable). The School also requires two final transcripts upon completion of any academic work in progress at the time of application. If applicable ensure that the conferment of a degree is shown on the final official transcript. Transcripts from secondary schools are not required. Transcripts are considered official only when in the original sealed envelopes. Official transcripts become the property of the University. Irreplaceable documents will be returned upon written request. Applicants who are not admitted or who do not accept offer of admission should note that any documents submitted will be held for one year only. UBC transcripts: it is no longer necessary for UBC students to provide transcripts. They will be downloaded for you. However, you must provide two official transcripts from all other institutions attended.
International transcripts: Applicants with non-Canadian transcripts should follow the instructions at this website: http://www.swfs.ubc.ca/undergraduate/international.htm). Applicants are required to submit two final official academic transcripts from every post-secondary institution attended. All documents must have original seals and signatures and must be submitted in original envelopes sealed by the issuing institution (not previously opened), unless it is an irreplaceable document. Transcripts must include those in the original language, as well as Certified English translations of all transcripts.
4. LETTERS OF REFERENCE
Applicants are required to arrange for two (2) reference forms (available on-line) to be completed and included (unopened) in the application package. If you have not received your references in time to meet the deadline, please include a note to say we will be expecting documents shortly. Photocopies and faxed copies are not acceptable. References should be from managers/supervisors from paid or volunteer social work experience, if possible. These reference letters will form part of the applicant’s evaluation. Applicants re-applying to the program are required to submit at least one new letter of reference. Please be sure to check your application package before sending, and enclose your post-card, so that you will know your package has been received.
FREEDOM OF INFORMATION Personal information provided on this application is collected pursuant to the University Act, R.S.B.C. 1996, c.468, as amended, and the Freedom of Information and Protection of Privacy Act, R.S.B.C. 1996, c.165. The information will be used for the purposes of admission, registration and other decisions on your academic status at the University. Information may also be provided to University student and alumni bodies. Information may also be used for research purposes, including research through the records of the BC Educational Records Linkage File. In all cases when used for research individual identities will not be disclosed.
