Nurse Appraisal Porter Brook Medical Centre Your annual appraisal provides the opportunity for you to consider, along with your appraiser, your professional development over the past year and consider your future development needs. Please bring with you last years appraisal, your current PDP, and your PREP portfolio. You may find it helpful to take this opportunity to review your practice in the light of the Code of Professional Conduct before your appraisal. The first section of this pack is designed to assist you in this. You will be offered the opportunity to discuss this at your appraisal. The second section considers your development within the context of the practice, with the aim of supporting you in your role. The third section is your PDP, which you can chose to complete during your appraisal with your appraiser, or at a later date. With your permission, this will be submitted to the Practice Manager and the staffing group along with a brief report of your appraisal. It will provide the basis for supporting your development needs throughout the next year. Your appraisal has been scheduled for …………………………………………. Please return the documentation to Anne by…………………… 1 Section 1. Reflections on Professional Practice. The NMC Code of Professional Conduct states that, as a Registered Nurse, you must: Respect the patient or client as an individual Obtain consent before you give any treatment or care Co-operate with others in the team Protect confidential information Maintain you professional knowledge and competence Be trustworthy Act to identify and minimise the risk to patients and clients. Your appraisal also offers the opportunity to review your PREP portfolio with your appraiser, and to include within it “evidence” to support your reflections. This could be items such as protocols you have been involved in developing, critical incidents, reflections on study days, letters of thanks from patients – these may refer to the work of the practice team, in addition to your personal contribution. 2 Relationships with patients As a Registered Nurse, you must: Respect the patient or client as an individual Obtain consent before you give any treatment or care Be trustworthy Maintain confidentiality Reflection: What do you think are the main strengths and weaknesses of your relationships with patients? How do you feel your relationships with patients have improved since your last appraisal? What would you like to do better? What do you think are your current development needs in this area? What factors in your workplace or more widely constrain you in achieving what you aim for in your patient relationships? Evidence to include in your portfolio: Examples of documentation could include: reflective practice accounts; information provided to patients about your services; any complaints, including your handling of it; appreciative feedback; patient survey data; significant event reports; minutes of meetings about patient services etc. 3 Working with colleagues As a Registered Nurse, you must: Co-operate with others in the team Reflection: What do you think are the main strengths and weaknesses of your relationships with colleagues? How do you feel your relationships with colleagues have improved since your last appraisal? What would you like to do better? What do you think are your current development needs in this area? What factors in your workplace or more widely significantly constrain you in achieving what you aim for in your colleague relationships? Evidence to include in your portfolio Examples of documentation could include; reflective practice accounts; description of team structure; minutes of meetings with attached staff; minutes of nurse meetings in the practice and the locality; information about problems which may have arisen between you and colleagues 4 Maintaining knowledge and managing risk As a registered nurse, you must: Maintain your professional knowledge and competence Act to identify and minimise risk to patients. . Reflection: What do you think are the main strengths and weaknesses of your clinical practice? How do you maintain and improve your knowledge and skills? How has the clinical care you provide improved since your last appraisal? What do you think are your clinical care development needs for the future? What factors in your workplace of more widely significantly constrain you in achieving what you aim for in the clinical care you offer? Evidence to include in your portfolio: Examples of documentation might include: reflective practice accounts; protocols you have helped develop; your PDP; practice achievements under QOF; brief reflections on study days/courses attended. 5 Section 2. Your development within the practice. Please use the following questions to help you reflect on your development within the practice 1. What do you consider to be your main achievements over the past year or so? (What do you enjoy about your work here and what do you feel you are doing well?) 2. Have there been any disappointments or obstacles over the last year? (Are there any area that cause you difficulties? Do you think any aspects of your work may cause problems for others?) 3. What are your aims and goals for the next year. (It may be helpful for you to think about your personal needs and the needs and wants of the practice as well as the national and local development needs - see Practice Development Plan included.) 6 4. Can you identify the support, help and development you may need in order to achieve your aims. (This may be changes in the practice or outside things like training courses.) 5. Can you identify any obstacles which need to be addressed in order for you to achieve your aims? 6. Are there any other issues you wish to discuss. 7.
