Procedure 1 Fill out this mail-in rebate coupon. 2 Attach the following supporting documents: a If you purchased your thermostats from a retailer, attach a copy of the receipt and all the original bar codes (UPCs) from the packages. b If you purchased your thermostats from a master electrician and/or had them installed by one, attach a copy of the invoice, making sure it shows his or her licence number along with the brand, model, price and number of thermostats. For thermostats installed in a multiple-unit building, the invoice must also specify the number of units in which thermostats were installed and the address of each unit (including apartment number). c If the thermostats were installed at a different address, please indicate it on a separate piece of paper. 3 Mail the coupon and supporting documents to the address below.
