How to fill in the form
Please complete the form electronically. Text boxes will expand as you type. Double click on check boxes to mark them. Name of education provider This refers to the name of the education provider that delivers the programme. Name of awarding / validating body This refers to the name of the education provider that awards the qualification. Name and title of programme This refers to the title of the programme which we will be visiting. If you are requesting a multi-professional visit, please complete a separate visit request form for each professional group of programmes you wish us to visit as these will be sent to different visitors depending on the profession. Mode of study The basis on which a programme may be offered e.g. full time, part time, other (please provide details). Duration of programme This refers to the length of the programme. Name of school / faculty This relates to the name of the school or faculty in which the programme has been placed. Annual monitoring date Please specify the month when your institution completes your internal monitoring process and signs it off as completed. Date of next periodic review / revalidation If you are aware of the date that your next periodic review or revalidation will take place, please specify here. Contact information Please provide the contact details for the Dean / Head of faculty / school; programme leader; and quality assurance officer associated with the programme. Please specify which of these contacts should be the main contact for the visit.

Visit information Type of visit There are different types of approval visit. An HPC visit is where we are the only organisation approving the programme. A joint visit is where a number of representatives, such as your internal validation panel, a professional body, the HPC or other regulators, are each approving the programme for different purposes. A multi-professional visit is when we review several professions in a single visit. For joint and multi-professional visits, please provide further information in the box below. Proposed student numbers This relates to the number of students you expect to undertake the programme each year. Proposed start date of next cohort This should include the date when you expect the next cohort to start. Academic year in which we have indicated that we will visit This should be the academic year which we have indicated in correspondence to you that we expect to visit. Three suggested dates for the visit Please suggest three dates when we can carry out the visit. If you are unable to arrange a visit within the academic year we have previously indicated to you, please provide details of your individual situation. We will endeavour to work with you to find a date appropriate to both of us. If you already have a British Psychological Society (BPS) visit scheduled and you would like us to visit at the same time, please indicate this on the form and we will endeavour to attend this visit. If you wish to hold a joint visit, with both the BPS and HPC but do not yet have a date confirmed with the BPS, please note that it is your responsibility to liaise with the BPS about their participation in the visit. Please note that we require six months’ notice of a visit, which cannot happen less than three months before the next cohort of the programme starts.
