We would like to take this time to say congratulations and thank you for considering us to be a part of your special day. Enclosed is the 4-page bridal information package we talked about during our conversation. Please take 35-40 min to look this over and fill out. It is important that both bride & groom have reviewed these pages and know what each other want for photos. Please fill out the Wedding Questionnaire (pg3), Bride & Groom Family Checklist (pg4&5) and the Traditional Checklist (pg6) as completely as possible. On the checklist please check if you just think that you may want them (this gives us an idea of what you may be looking at). Only answer the questions you know, we will discuss the rest during our appointment. Please use black or blue ink when filling out these forms. Photocopy and mail originals of these 4 pages. It is important for you to keep a copy should anything need to be discussed via phone; you will be able to see what we are looking at. Do not fax this back to us, it may be very hard to read your information. In order for your custom estimate to be completed in a timely manner, the 4 checklist pages should be returned to us by_________________. (If you receive this letter past our requested reply date, you must e-mail or call us ASAP!) There are only 52 weekends in the year and they fill up very quickly. Dave turned down over 100 weddings last year; we work on a first come first serve basis. Once paperwork is received, Dave can complete your custom estimate.
