SURVEY INVITATION Dear Permittee/Consultant, Thank you for your participation in improving the air permit process. Enclosed please find a survey to assist ADEC to better serve you and your Air Title V Operating Permit needs. This survey invites you to provide information on how you, the regulated community, would like your permit(s) structured. Please take the time to fill out this survey and return it to ADEC by August 15, 2008. You can also download and save a MS Word copy of the survey through our website at http://www.dec.state.ak.us/air/ap/mainair.htm. Your involvement benefits both you and the state Title V Operating Permit Program. By working together, we can improve our permit process and provide better service to you, the regulated community. Please feel free to include comments if you have alternative suggestions or recommendations. If you do provide additional comments, please include in your response your contact information in case ADEC needs clarification on your comments. The permit should be organized by emission unit with the applicable regulations and requirements listed for each emission unit. The monitoring, record keeping and reporting requirements would be listed or referenced with each unit’s permit terms. If the permit is organized by emission unit, the stationary source wide requirements would be first in the permit followed by the emission unit specific requirements. The permit should be organized by regulation and applicable requirements (current permit style). The permit should include a table that organizes the monitoring, record keeping, and reporting requirements included in the permit. This table would not replace the permit condition language currently seen in the permit.

The permit should contain a table of all the applicable and approved test methods required for the permittee to determine compliance. The department should develop an annual compliance certification (ACC) form unique to each permit. There would be some cost associated with this service for development and revision as necessary. After the permit issuance, the department should meet with the permittee to discuss permit terms and conditions, monitoring, record keeping, and reporting requirements (MRR). The Statement of Basis should include all public comments and the department’s response.
