1720 Control of discharge of oil 8. C/E reported serious deficiencies to company as per list enclosed (report of deficiencies consisting 43 items). Many of them being serious in nature. Further, the following deficiencies are identified during inspection. - Oily water separator, oil trace in discharge line overboard from cock, Incinerator not in working condition, - Funnel plate holed, - Heavy rain shower flow in to engine room. In view of the above serious deficiencies, the PSC visit has been abandoned. Details of deficiency 1. When entering the ship by gangway portside, no safety net in use (unsafe), gangway corroded (over painted) 2. Several fire doors in alleyway not closing properly by self-closing device and/or locking device, e.g. main deck to stairs, from bridge to stairs. 3. Stbd lifeboat embarkation ladder, rope is gone / deteriorated 4. Stbd air vent on 2nd deck holed / corroded 5. Found at least one hole on aft 2nd deck Stbd side near air ventilation from engine room 6. Weather tight door engine room funnel, PS, impossible to close, because big electrical cables obstructed this possibility.. 7. Vent hood Stbd side next to funnel bottom part of it is corroded. Instead of 5 crew as indicated in safe manning need to have the certificate II/4 (AB + OS), only 3(AB + OS) crew member have one.

Details of deficiency 1. Vessel's No.1 steering gear pump is inoperative, due to burnt out magnetic contactor in electrical panel. Vessel lacks redundant steering capabilities as required by SOLAS. In the event of failure of the No.2 steering gear pump, vessel would be unable to navigate safely , in that the steering gear would be rendered inoperable. 2. Pre-arrival tests entered in bridge log do not reference inoperative No.1 steering gear pump. (70: Rectify deficiency prior to next US port after sailing foreign.) 3. Non Tank vessel response plan does not contain sections 1-3, 10 and 11. VPP approval was provided indicating plan has met USCG requirements and vessel does have the complete copy..

Vessel failed to follow the Safety Management System with regard to reporting leaking shaft seal, holed lube oil line on emergency generator, damaged windows, holes in marine deck and inoperable OWS as required by vessel's SMS.
