Dear ~[Title] ~[Surname]

A few weeks age we sent you a letter inviting you for your FREE NHS Health
Check. It appears you have not yet taken advantage of this.

I would strongly encourage you to come along for this valuable check up. People
are often unaware that they have developed conditions such as high blood
pressure, heart disease, stroke or diabetes until it’s too late.

The check is quick and easy, involving questions and measurements such as
age, family history, height, weight and blood pressure. There will also be a
simple blood test to measure your cholesterol (see overleaf for further details).

If you do have a problem we can provide treatment and offer you a chance to talk
to people who can help you plan for a healthier future.

Please contact the practice on ~[telephone number] as soon as possible to
arrange your FREE NHS Health Check.

If you have any questions please feel free to phone and ask to speak to one of
our practice nurses who will be happy to give you further details.

The check only takes about 20 minutes but could add years to your life.

Yours sincerely
