Enclosed is our check payable to EXCELLENCE IN PRESENTATIONS in the amount of $150 times the total number of reservations requested above ($750.00 per person listed on Alternate Airport Options form). We understand that the only acceptable forms of payment are U.S. money orders, bank cashier’s checks, an FFA Chapter check, or a check drawn on our high school or district accounts. Certification by Chapter Advisor: The following must be completed by the Chapter FFA Advisor for reservations indicated on the first page of this form to be accepted. The FFA Chapter Advisor must understand and agree to all regulations regarding these trip arrangements. This agreement must be indicated before reservations will be accepted. To so indicate, advisor must read the following regulations, initial EACH ITEM at the left of the statement and sign the form at the bottom of the page.
Advisor's Initials:

1. ALL reservations and subsequent participation is between the local FFA chapter and Excellence in Presentations. Students, parents, or others connected with a local chapter may NOT directly request space with Excellence in Presentations. 2. ALL information and dealings relative to participation on this trip are to be between the local FFA Chapter Advisor and Excellence in Presentations. Students, parents, and others should not contact this office to request information, arrangements, etc. Such contact will be referred back to the local FFA Advisor. 3. ALL payments and obligations relative to this activity are between the local FFA Chapter and Excellence in Presentations. Dealings regarding payments, refunds, amounts due, etc., will not be discussed with students, parents, boosters, etc. Chapter advisors are responsible for all such discussions. 4. Payments received from local chapters by Excellence in Presentations will not be recognized as being paid by or for any specific individuals. Excellence in Presentations will only concern itself with the TOTAL financial obligation of the chapter for ALL of its participants, and payments received will be recorded against that TOTAL debt. 5. ALL deposits made for reservations are ABSOLUTELY NON-REFUNDABLE for any reason. Requests to change a reservation from one trip to another shall be considered a cancellation of the original reservation, thus forfeiting the original $150 deposit. 6. On August 7, 2009, names and specific requested information as well as the total balance owed is due for each reservation being held by the chapter. Any information not in the office of Excellence in Presentations on that date shall constitute cancellation of the space reservation and therefore, forfeiture of the deposit(s). 7. ALL trip information will be mailed to the FFA chapter advisor or other local vo-ag instructor. No information will be mailed directly to participants’ home addresses. The local FFA Advisor is obligated to keep participants informed and secure required forms. Information will be mailed to the teacher and address named on the first page of this form between June 1 and September 1, 2009. After September 1, 2009, all information will be mailed to the FFA Advisor at the local high school. 8. ALL participants are obligated to abide by a strict set of rules and code of ethics during this activity. Chapter advisors should only recommend participants of high caliber who will behave properly at all times and be a credit to the FFA. The local advisor understands tha t rule infractions will result in punishment which often includes having the participant sent home early. Should this happen, the local FFA Chapter agrees to pay the cost of the participant's early transportation home and recognizes that NO pro-rated payments are due back to the chapter. 9. ALL participants registered on state association-arranged group trips to the National Convention and related activities must travel on the published group itinerary as originally arranged with Excellence in Presentations. Absolutely no deviations from the originally prepared travel plans can be accommodated for any reason. Any participant who must deviate from the group itinerary (i.e., go a day later, come home a day earlier, etc.) must do so by arranging alternative transportation at his/her own expense. The FFA will not accept supervision responsibilities for such a deviate. Travel on alternate dates and from alternate airports can be arranged in advance. But once booked, travel itineraries cannot be changed. 10. Chapter Advisor understands that all prices quoted for travel packages is based on quadruple occupancy for students and double occupancy for adults. Requests for alternate room arrangements will be granted IF space is available and the local FFA Chapter agrees to pay any additional costs associated with said special requests. 11. Payment for the requested reservations is enclosed with this form at the rate of $150 times the total number of requested reservations. The check is payable to EXCELLENCE IN PRESENTATIONS and is a U.S. money order, bank cashier's check, an FFA Chapter check, or a check drawn on school or district accounts. If the chapter has requested Alternate Airport Options, a deposit of $750.00 per person requesting the alternate arrangements is enclosed. _______ 12. Due to the volatility of current fuel prices, taxes, and government fees, the airline and bus companies, in providing price quotes for this group travel, have withheld the right to impose a fuel surcharge in the event of significantly increased fuel prices, and/or pass along new government fees. Should the airline and or bus companies impose such a surcharge, taxes, fees, etc., the local FFA Chapter Advisor acknowledges that the FFA Chapter will pay these additional costs for its reservations shown on page one of this form. By my signature below, I hereby certify that I have read and understand all information included in this packet relative to the 2009 National FFA Convention trips as well as all regulations specified above and that our local FFA Chapter agrees to abide by all stated regulations.
