‘ To secure sustainable services in Hull and the East
  Yorkshire through Health and Social care organisations
  co-operating to transform and integrate services to
  provide high quality seamless care for patients’

  Three key objectives that support the transformational
  programme to:
 improve quality , secure ,sustain and transform
  organisations and services to meet the challenge of
  improving productivity (by 2015)
 transform services through integration across the health
  and social care system whilst ensuring organisational
  financial sustainability
 Re-design services to meet our public and patients needs
  through engagement and involvement

Initial Focus
 Identified areas for focus with the key criteria:
    requiring transformational change
    requiring whole system collaboration and co-ordination
    potential to realise savings with improved operational
    potential for increased demand on services by 2015
    potential to shift care into an appropriate setting
    existing whole system working already in place
    opportunity to improve quality and patient experience
