									List of documents required


                                                 I. PERSONAL DETAILS
       signed mortgage application form
       copies of 2 IDs (passport, card of residence, driving license, other document with photo)
       bank account statements for the last 6 months with balances at the end of the month
       credit card account statements for the last 3 months (optional)
       details of existing loans (mortgage & other loan statements showing payments over the last year)
       residency card and permission of the Ministry of Internal Affairs and Administration (if applicable)
       certificate of separate estates in matrimony (if applicable)
       valid judgement on divorce (if applicable)
       copies of relevant pages of military ID (if applicable)

                                            II.     INCOME DOCUMENTS
II.A     Employment contract
       employer’s certificate (bank form) + payslips for the last 6 months
       declaration of not conducting business activity (bank form)
       current P60 / W-2 / other personal tax return
       employment contract (required when the Borrower has the total length of employment shorter than 3 months
        employment period at the present employer)

II.B      Business activity
       registration documents
       relevant permits and licenses (if applicable)
       Revenue Office statement (or statement certified by a Chartered Accountant, Certified Personal Accountant,
        etc.) about non-default with payment of taxes, insurance premiums
       Revenue Office Statement confirming income over the last 12 months (year-end tax certificates)
       Financial documents proving monthly income and company performance (certified by a Chartered
        Accountant, Certified Personal Accountant, etc.)

II.C      Self-certification ("Asset-based" offer)
       net worth summary confirmed by a Chartered Accountant, Certified Personal Accountant, etc.
       document certifying ownership of the real estate (e.g. notarial deed)
       relevant documents listed I section II.A and II.B to confirm income which cover at least the monthly
        repayment

                                          III      PROPERTY DOCUMENTS
       transcript from the mortgage register (both collateral and object of lending)
       certificate issued by the bank employee confirming the inspection was carried out + photographs illustrating
        the neighborhood and building outside
       valuation of the property


III.A     Second-hand property purchase / apartment, house
       preliminary purchase contract with the vendor / declaration of the vendor (bank form) / declaration of the
        buyer (bank form)
       document confirming a basis for the purchase of the property by the current owner
         certificate issued by the Housing Co-operative (containing information on no arrears with payments and
          specifying the person entitled to the co-operative member’s right to its own apartment [SWPdL])
         permit to buy a property in Poland (if other than apartment)


III.B       Property purchase / off-plan investment

         preliminary contract with the developer / reservation contract with the developer / declaration of the developer
          (bank form) / declaration of the buyer (bank form)
         transcript from a relevant register of the developer
         constructional permit final decision
         authorisation for the developer / Housing Co-operative (if the developer is represented by third parties)
         excerpt from the mortgage register of the land on which the investment is implemented
         excerpt and drawing from the land register and decision on development conditions
         declaration of no mortgage register for the apartment and no contraindications to separate the same (in the
          case of a Housing Co-operative)
         disbursement schedule (for loans disbursed in tranches)
         consent to postpone the payment date
         consent of the Developer’s Bank to free of charge separation of a mortgage register for the apartment (in the
          case of entries contained in section IV of the mortgage register)


III.C        Fit-out / redecoration / refurbishing
         fit-out characteristics (bank form)
         valid building permit (if applicable)


III.D       Purchase / building plot
         preliminary purchase contract with the vendor / declaration of the vendor (bank form) / declaration of the
          buyer (bank form)
         document confirming a basis for the purchase of the property by the current owner
         transcript from the real estate register (architectonic documentation)
         land development plan on a geodesic map
         development conditions / excerpt from the local land development plan
         permit to buy a property in Poland

III.E       Acquisition of rights under an agreement for building real estate
 agreement on assignment of rights under an agreement on building housing real estate (signed and sealed by
        the developer or the developer is a party to the agreement)

III.F       House building / completion
 valid building permit
 excerpt and drawing from the land register / decision on division, if the plot was separated from a bigger piece
        of land and such division was not recorded in the excerpt from the land register
       development conditions / excerpt from the local land development plan
       land development plan on a geodesic map
       house design
       building log-book (if the building permit is more than two years old)
       building works estimate with time schedule

IV         Additional documents
         Power of Attorney empowering third party to act Applicants’ behalf (if applicable)
