Pelham Parks and Recreation AFTER SCHOOL SKI/SB PROGRAM 2010 With Pats Peak in Henniker, NH Open to students in Grades 6 – 12, Pelham Memorial and Pelham High schools GENERAL INFORMATION PLEASE READ AND KEEP THIS INFORMATION. PARENTS ARE ALSO RESPONSIBLE FOR THE INFORMATION PROVIDED IN THE PATS PEAK APPLICATION FORM. Call us at 635-2721 with any questions or email Recreation@pelhamweb.com. Forms may be mailed or delivered between 8 a.m. and 4 p.m. Mon-Fri to our office at 6 Village Green. Payment must accompany form for it to be accepted. NEW THIS YEAR: You may sign up online at Pats Peak for their program and use our Webtrac online (see P&R website link) to pay costs, both with MC/VISA. DATES: WHERE: TIME: Every Thursday beginning 1/07/10 through 2/04/10 (5 week program) Pats Peak, Henniker, NH - Please note: ***No school, no ski*** Bus arrives at Memorial @ 2:10 p.m. and PHS @ 2:30 p.m. Memorial buses return 8:30 p.m. to Memorial parking lot; PHS bus returns 9:00 p.m. to PHS parking lot. (NOTE: It is possible that one bus will carry both Memorial and PHS studentsin this event, that bus will return at 9:00 p.m. to PHS). Our registration is limited by the number of students we can fit comfortably on the three Coach buses we obtain. First come, first served. Prices on Pats Peak application do NOT include transportation/costs; see separate cost sheet! If you’ve registered your child online with Pats Peak, the $125 costs is due Parks and Rec and can be paid with check (payable to Town of Pelham) or you can pay online at https://webtrac.pelhamweb.com with MC/VISA. Each applicant MUST ATTACH TWO CHECKS with paper clips to the application, if not registering or paying online. No staples please! See separate cost sheet for breakdown and parent letter for details. Two checks – one payable to Pats Peak for cost of program on their application and a second check payable to Town of Pelham for bus and costs. Payments in full must be made at the time of application; no deposits. NO CASH REFUNDS per Pats Peak. There are a few scholarships available for qualifying applicants. Ask at recreation office. You may purchase a cafeteria card from Pats Peak. See the ski/sb application form or website for details or other items such as mittens, goggles, café cards, etc. Students should otherwise bring $ to purchase food/drink items at the Pats Peak cafeteria. SPACE: COSTS: PAYMENT: FOOD: APPLICATION: One original application needed per student (no copies accepted!). Please note that each Pats Peak original application MUST BE SIGNED BY THE PARENT. Applications without signatures will not be accepted Please take time to carefully complete the form as instructed before returning it. Tear off the last page for our office to retain; keep the informational front pages. We will also require a new emergency medical card in triplicate on each student by 12/15/09; please obtain from our office or on the website. 1 DEADLINE: NO APPLICATIONS can be accepted after November 24, 2009, or sooner, should the program fill to capacity. The number of participant’s IS LIMITED. First come, first serve. We are restricted by the number of students who can fit on the buses booked. Please reserve at time of registrations; $25 late fee after and on availability basis. Each student who is registering for ski or snowboard (you must choose one or the other) must be at a Level 5 to enter the program without lessons. If your child will not have a lesson, you must circle the X; if you choose lessons, circle a number that fits (see chart inside application). Lessons ARE required for all beginner skiers and snowboarders. They are typically one hour around 4:30 pm. Free ski/snowboard time is available for the rest of the evening at Pats Peak. One triplicate emergency medical card is REQUIRED for each participant. Please obtain and complete the form from our office. No child will be allowed on the bus without this triplicate card in our possession! Correct easy to read contact numbers are essential. Pelham Parks and Recreation does provide a letter to each family who has a child or children registered in the After School Ski program at registration. It includes information on passes, bonus days, clothing, etc. at that time. Please be sure you pick one up with application and any other separate notices that apply. No further notice(s) given after registration. Photos or videotaping may be taken and included on our website. WE ARE LOOKING FOR CHAPERONES! Please let us know if you and able to do so. Chaperones are invited and urged to attend a meeting on Tuesday, December 15 2009 at 6:30 pm in the Pelham Police community room with our director and/or coordinator(s).
