WSLH PT's prices are competitive with all of the major PT providers, and often-
times lower. We also have a pro-rated pricing system that allows customers to
enroll throughout the year and only be charged for PT samples received from
enrollment through the end of the calendar year.

How Do I Enroll?
Current Customers: Review this catalog and edit your Re-Enrollment Form as needed.
New Customers: Fill out the enclosed WSLH PT Order Form using the steps below as a guide. Order Forms are also
available on our website [www.wslhpt.org], or call our friendly customer service staff at 800-462-5261 to have a form
faxed or mailed. If you are also interested in PTPlus products (on-line training/competency or VeriSamp), fill out the ad-
ditional order form(s) for those services.

Page 1 - Demographics
1.   SL NUMBER: New customers leave this blank; a number will be assigned when the order is processed. If you are a
     returning customer and wish to use your “old” SL number, please fill it in.
2.   SHIP TO: Address where PT samples should be sent. You must list a street address. Couriers do not deliver
     packages to a PO Box. Include phone/fax numbers and a business e-mail address.
3.   REPORTS TO: Address where mail should be sent. If there is no mail receptacle at your street address, please
     provide a PO Box number. Include phone/fax numbers and a business e-mail address.
4.   BILL TO: Address where invoices/bills should be sent (e.g. Accounts Payable, office manager). Include phone/fax
     numbers and an e-mail address if available.
5.   METHOD OF PAYMENT: You will be invoiced within 30 days of order receipt. It is NOT necessary to include
     payment with your order. For facilities that require a Purchase Order number for orders, please list the PO# here.
     Purchase Order numbers will be printed on the invoices.
     We accept Visa or MasterCard. For security and audit reasons, please wait for your invoice to arrive and then call the
     phone number on the invoice to provide credit card information.
     You are encouraged to fax your order as soon as possible to 608-265-1111.

6.   MANAGEMENT SERVICES: If your laboratory has a consultant, lab director or POCT coordinator who needs a copy
     of the reports sent to the person listed in the “Reports to” section, provide their information here. Consultants/POCT
     coordinators responsible for multiple sites can select free group re-enrollment and/or confirmation services to help
     them manage the PT enrollment process (see page 20 for detailed descriptions).
7.   LAB TYPE: Please indicate the type of site doing this testing.

Page 3 - Accreditation Information
1.   CLIA I.D. NUMBER: This is your identification number under the Clinical Laboratory Improvement Amendments of
     1988 (US labs only).
2.   ACCREDITATION AGENCY: Indicate the agency that performs on-site inspections of this site and include any ID
     number (other than the CLIA ID) which is associated with this accreditation. This insures that PT enrollment records
     and scores will be sent to the correct accreditation agency.
3.   DESIGNATION OF PRIMARY PT PROVIDER: Customers are required to select the appropriate option. If unsure
     which option is valid, please call 800-462-5261, ext. 45 for guidance.

            Special Note for customers accredited by the College of American Pathologists’ Laboratory Accreditation Program
                  Please refer to information on pages 8-14 to ensure enrollment in our PT program will meet
                                     accreditation requirements for your activity (test) menu.
1.    ORDERING: Indicate the PT products needed by putting the number of sets in the adjoining box. One “set” includes
      all shipments for the calendar year (normally 2 or 3). Facilities with more than one instrument/kit doing the same
      testing may need to order multiple sets or use “X”, “Y” or “Z” options (if available) to cover their needs.
              Product Codes ending in “-X” provide an additional set of samples for testing. This option is useful for
              in-house documentation of training/competency, provides a convenient back-up for breakage/spillage or can
              be used to troubleshoot problems after PT reports are received. “X” products do not allow reporting of
              additional results.
              Product Codes ending in “-Y” provide the ability to report another set of results. No additional samples are
              included. Additional result sets reported must operate under the same CLIA ID#.
              Product Codes ending in “-Z” will provide both an additional sample set and the ability to report additional
              results.
          Please Note: “X” and “Y” products must be ordered in conjunction with one base (-R) product.
2.    SUB-TOTAL: The amount of all PT products ordered.
      Note: Prices are divisible by the number of yearly shipments. Customers enrolling after the first of the year will be
      charged only for the number of shipments they will receive from order process date through the end of the calendar
      year.
3.    PTPlus PRODUCTS: Binders and the Quality Management Report should be listed in this section. Online
      competency/training and VeriSamp validation sets have separate order forms. Transfer any amounts from these
      forms if all services should be billed with the proficiency testing order.
      Note: Pro-rated pricing (see #2 above) does not apply to PTPlus products.

4.    TOTAL: Add the sub-total, annual enrollment fee and any optional services (e.g. binders, PTPlus products) ordered.
             Samples must be ordered well in advance of the first shipment. To assure your lab’s participation
                             in all shipments, 2009 orders must be received by Dec 1, 2008.


     PORTAL ACCOUNTS: Customers with online access can request to have portal accounts set up for any or all staff.
     Two account types are available: single site and multi-site.
     Portal accounts provide customers the following options:
                 View/download/print PT reports from an online Reports Catalog. The catalog stores over two years of
                 reports for most specialities (five for blood bank).
                 Enter PT results online. Two levels of permission are available.
     If interested, please visit our website and click on the “Request an Account” link under the teal CUSTOMER LOGIN
     button. Review the information, download, fill out and submit the appropriate Portal Account Request.

Shipping Policy
Two-day shipping via courier is included in the price of each program. Additional charges for special services (e.g. over-
night) or shipment outside the 48 contiguous states (e.g., Hawaii, Alaska, Canada, etc.) will be applied. Import permits for
international labs must be provided by customers at least one month prior to first scheduled shipment.
PLEASE NOTE: Dates listed in this catalog are for general reference only! A final shipping schedule will come with your
enrollment confirmation letter.


Enrollment Confirmation
You will receive a confirmation letter, certificate, policy information and final shipping schedule within three weeks of
receipt of your order.


Cancellation Policy for PT Programs
Full refunds will be made for program cancellations requested more than 30 calendar days before a program’s first ship-
ment date. Partial refunds will be made for cancellations requested at least 30 calendar days before any subsequent ship-
ments. Refund checks, if requested, will be issued within four to six weeks. Since annual active enrollment status must
be maintained, we will not rebate a pro-rated fee due to temporary suspension of testing. The annual enrollment fee is
non-refundable.


Replacement Policy
If samples are missing or damaged in transit, please call (800) 462-5261, ext. 0, or (608) 265-1100, ext. 0 to request replace-
ments. Requests for replacement samples must be made within the time frame noted on the instructions. Replacement re-
quests due to mishandling of samples (misdilution, spillage, etc.) on the part of the participant must also be made within
the specified time frame. A replacement fee will be charged for samples which have been mishandled by the participant.
This fee will cover the cost of the samples, shipping and handling.



 How WSLH PT Can Help You Meet Accreditation Requirements
 Participation in proficiency testing is required for analytes listed in CLIA ’88 and analyzed by non-waived methods.
 These analytes are printed in black ink in our catalog. For non-regulated analytes (those printed in teal ink), CLIA
 dictates that labs must have a quality assurance plan that establishes accuracy and reliability at least twice/year. A
 convenient way to accomplish this is by subscribing to a PT program that monitors those analytes.

 Facilities accredited by agencies such as CAP, The Joint Commission or COLA, or that are located in certain states, may
 be required to enroll in a proficiency testing program for CLIA non-regulated analytes and/or testing performed by
 waived methods. WSLH PT provides cost-effective PT programs for this purpose. Check with your accrediting agency
 for further clarification.

 Most of the analytes in our catalog are accepted by the CAP's Laboratory Accreditation Program. A complete listing of
 these analytes can be found starting on page 9. A much shorter “not accepted” list is provided on page 8. Analytes not
 accepted are marked by a purple " ** " in the catalog listings.

 WSLH PT has expanded our services beyond proficiency testing to assist you with other accreditation requirements.
 VeriSamp validation samples assist in method verification and training. Online training and competency (both lab and
 PPMP) services provide time-saving, cost-effective systems for tracking and documenting these vital functions. The
 quality management report allows timely access to PT data for troubleshooting and follow-up.

 WSLH PT has always been known for its friendly and helpful customer service staff and knowledgeable program coor-

Important Information for CAP Accredited Customers
Most of the analytes listed in our catalog are accepted by the CAP's Laboratory Accreditation Program (LAP) to fulfill PT
accreditation requirements (please refer to the CAP PT Enrollment Guide for a list of analytes which require PT). The full
list of analytes accepted by the LAP follow this page.

Current and potential customers should carefully review the information below before placing their order with WSLH PT.

The LAP requires three events of data, low percentage of not-scored and a minimum group size before accepting analyte
scores from any alternate PT provider.

                       Online Training and Competency
WSLH Proficiency Testing is pleased to offer to our customers the Medical Training Solutions (MTS) web-based training
and competency assessment products. These award-winning products have been developed by MTS and the University of
Washington Department of Laboratory Medicine and cover a wide range of subjects pertinent to the clinical laboratory.

The Training Library covers a variety of safety, pre-analytical and analytical topics. There are also two Competency As-
sessment products: Laboratory and Provider Performed Microscopy Procedures (PPMP). Both of these product lines offer
an image-based, quick, ten-question format with new test question sets every six months.

E-mail notifications to staff and printable performance reports for administrators are a couple of the time-saving features
which make MTS the type of quality, easy-to-use service that would meet the needs of WSLH PT customers.

Lab managers, technical consultants and POCT coordinators are often physically removed from the testing sites for which
they have responsibility. WSLH PT offers services which we believe make the entire PT process more efficient.


Copies of PT Reports                                                                         No additional cost
Copies of all proficiency testing reports sent to the enrolled site(s) along with statistics and event summary notes can be
sent to a technical consultant, lab director, manager or POCT coordinator if the information is supplied to us. These copies
will be mailed at the same time reports are sent to customer labs.

Group Re-enrollment                                                                          No additional cost
This service is intended to streamline the re-enrollment process … and it’s free! Re-enrollment forms for all sites can
be combined in a single mailing directly to one individual rather than each of the lab sites. The sites can be in the same
healthcare system or in different systems under the oversight of a consultant. This saves valuable time in tracking re-en-
rollment and ensures that all sites will be enrolled in proficiency testing for the new year.

If you are a current customer responsible for multiple sites and would like to take advantage of this service, please call
our customer service number – (800) 462-5261, ext. 0. If you are new to WSLH PT, please check Group Re-enrollment under
the Report Copies section on page 1 of the Enrollment Form.


Enrollment Confirmation Copies                                                                No additional cost
Testing sites are expected to keep enrollment confirmation records on site for at least two years. However, managers and
consultants usually have responsibility for organizing documentation prior to accreditation inspections. By checking the
Enroll Confirm Copy option, copies of the enrollment confirmation forms (including site-specific shipment schedules) are
provided to assist you with this task. Many consultants receiving this service find it also “closes the ordering loop” and
assures that the purchasing department has submitted all orders and we have correctly processed them. This service is
offered at no charge.


Quality Management Report (QMR)                                                                        PRICE: $400
Every three months you will receive an electronic report (Excel file format) containing all PT testing information for your
sites covering the most recent five quarters (Current customers will have their data pre-populated into their first report).

The report will be sent to your e-mail address and comes pre-sorted into four categories:

        Sheet #1 – all PT testing data sorted by WSLH PT customer number

        Sheet #2 – analytes by site/year/event with unacceptable scores

        Sheet #3 – analytes by site/year/event with acceptable scores but under 100%

        Sheet #4 – analytes receiving 100% for non-consensus, not scored or excuse requested situations

Scoring details such as “Late” or “No results received” will be included in the report to help you identify and document
the reasons for less than satisfactory performance.
