RFQ 440-05-00-0093 Georgia Department of Labor/ Roosevelt Warm Springs Institute Cementeous Ceiling Removal at Georgia Hall Kitchen and Dining Room Phase 1 Specifications SCOPE OF WORK Georgia Department of Labor, Warm Springs Institute for Rehabilitation requires the removal and disposal of cementeous ceiling from Georgia Hall Kitchen and dining room. These cementeous ceilings are deteriorated and are a safety concern to employees, residents and visitors. The Kitchen and Dining Room ceiling area is approximately 4000 square foot. DEMOLITION REQUIREMENTS The work under this contract shall consist of the following but not limited to the following: 1. The contractor is responsible for the removal of the entire cementeous ceiling from Georgia Hall Kitchen and Dining Room. Contractor shall protect all the electrical conduits, fiber optics and phone lines and fixtures during the demolition. The contractor will be responsible for repairing and or replacing any damages. Contractor shall protect all sprinkler lines and sprinkler system heads during the demolition work. The contractor will be responsible for repairing and/or replacing any damaged system. Contractor shall protect all HVAC duct and HVAC systems during the demolition work. The contractor will be responsible for repairing and/or replacing damages. Contractor is responsible for removing all crown moldings with care and place in storage furnished by Warm Springs and under the supervision of the engineer. All crown molding shall be identified and inventory through a numbered system for future reuse and exact location. Contractor shall define the methodology of the removing the ceiling as a part of the bid. All rubbish, debris etc., from demolition work shall be removed from the premises during and/or upon the completion of work, leaving the site area acceptable to the satisfaction of the Georgia Department of Labor engineer. Contractor shall furnish to the D.O.L site engineer all manifest of demolition material from the dumpsite. Contractor shall take whatever steps necessary to control dust during demolition and removal of cementeous material. The contractor will monitor the haul route for debris. Contractor shall exercise special care not to damage the ceiling insulation. SUBMITALLS: A. B. Submit dismantling and removal procedures and schedule. Reuse Management Plan: Before the start of demolition, submit for approval by the Georgia Department of Labor engineer the Reuse Management Plan for all materials designated to be reused. Submit landfill-tipping receipts for all debris. Submit inventory plan of items, which will be use for reuse. QUALITY ASSURANCE: C. A. Regulatory Requirements: Comply with all applicable requirements of the State of Georgia and applicable local ordinances and regulations concerning management of demolition and all subsequent modifications and amendments to the same. Pre-Construction Reuse Management Conference: Prior to the beginning of Work at the site, D.O.L will schedule and conduct a conference to review the Materials Reuse Management Plan and discuss procedures, schedules and specific requirements for materials to be reuse. Discuss and coordinate all the work staging between the Contractor and Warm Spring management to make sure the contractor’s performance will not slow down the day- today operation Warm Spring. 1. Attendees: The Contractor and his associates for this project, including personnel to be in charge of the material handling (reused products); and staff from D.O.L authorized by project engineer. B. 2 RFQ 440-05-00-0093 2. Plan Revision: Make any revision to the Reuse Management Plan and present to project engineer. Engineer will review the revision and inform the contractor on his decision and approval with 48 hours. C. Implementation: Contractor shall designate an on-site supervisor responsible for instructing workers and implementing the Reuse Management Plan. Distribute Reuse Management Plan to the job site foreman and each appropriate subcontractor. Provide on-site instruction on appropriate salvaging, removing, recovering and handling procedures to be used by all subcontractors at appropriate stages of the work at the site. STORAGE AND HANDLING: A. Site Storage: Remove all materials that are to be reuse from the demolition site to an approved location designated by the Owner. If the material is stored, a secure and protected location shall be created where the salvage materials will be secure from damage and thief. If the material is of extreme value, the Contractor or the Owner shall specifically include these items on this Builder Risk or Property Insurance. Handling: Contractor is responsible for ensuring all material to be reused is removed and handled reuse with care. The reuse material must be free from damage, dirt, adhesives, solvents or other contaminates that may come in contact with them. SCHEDULES A. The following materials, at a minimum, shall be reused, salvaged recycled or recovered: 1. 2. Millwork, crown molding and other similar interior finishes Electrical fixtures B. PROTECTION OF EXISTING FACILITIES The successful bidder shall, as soon as he receives a Notice to Proceed for the work, enter the premises and mobilize in order to start this project. Contractor shall exercise all Warm Springs/ Department of Labor safety and security rules and regulations. The contractor shall protect all existing equipment, property, furniture, etc., from damages that may occur from demolition work. All ceiling and wall structure affected by demolition work shall be kept weather-tight. The 3 RFQ 440-05-00-0093 contractor shall provide all bracing, tarps, temporary partitions, barricades, and/or other safety means deemed necessary by the Georgia Department of Labor engineer for the protection of existing facilities and Warm Springs personnel. Upon completion of the ceiling removal, two layers of three-millimeter polyethylene plastic shall be stapled to underside of the roof truss/bottom chord as a temporary means and measure to allow HVAC function after the contractor completes the work. Due to the location of this work no heavy equipment (bobcat and/or tractors) will be allowed in this area due to access restraints. OWNERSHIP OF PROPERTY No right, title property or interest of any kind whatsoever in or to the land or premises upon which such buildings or structures stand, is created, assigned, conveyed, granted, or transferred to the contractor, or any other person or persons, except only the license and right of entry to remove the cementeous ceilings located in Georgia Hall kitchen and dinning room in strict accordance with the Contract. All salvage becomes the property of the contractor, but storage of such materials of the project area will not be permitted except for the duration of the contract. Contractor shall dispose all construction debris at a predetermined location. Any demo material and/or property the contractor deems salvageable from the project area will and wants to retain for their possession shall request from the site engineer prior to removal from site. Failure to comply with this may void the contract immediately Contractor shall dispose all construction debris in an approved dumpsite off the Warm Springs premises. Personal property of third persons or of occupants of building on the site shall not become the property of the contractor. 4
