Appendix A ONTARIO STUDENT OPPORTUNITY TRUST FUNDS (OSOTF) Introduction The “OSOTF” awards refer to a class of awards, which have resulted from Ontario government‟s “matching” program. Under the program, every dollar of donation received for student assistance has been matched by the government as well as the University on a dollar-for-dollar basis. The OCI/PMH Foundation has contributed funds to this program, and as a result, we can now use the annual interest to assist graduate students at the OCI. All students at OCI who are registered in a graduate department at UofT and whose supervisor has an OCI appointment are eligible to apply. These funds have to be administered through the Dept of Medical Biophysics by the Faculty of Medicine. In order to access them certain conditions must be fulfilled. a) Recipients must be Canadian citizens or permanent residents living in Ontario with an Ontario address. b) Recipients must demonstrate need. The OCI awards committee has defined „need‟ for a graduate student as an income of $27,900 or less. c) Students have to apply for these funds. Please fill out the attached Financial Needs Assessment Form and return to Carolynn Mar by June 1st, 2009. If you have already been assessed for the Ontario Student Assistance Program (OSAP) or the University of Toronto Advance Planning for Students (UTAPS), complete sections 1, 2, and 3 only. Please note that the Financial Needs Assessment Form asks for earnings for the year 2009/2010. For the purpose of this form, please assume that no funds will be coming from your supervisor. For example on page 2 of the form, under Resources, you should list all sources of income (except for any grant funds that you expect from your supervisor -–see note 2 on the assessment form) including award monies and the source of the award, for example: CIHR - $21,000; OGS - $15,000, etc. Please note that the amount that you receive from the OSOTF will form part of your overall stipend and this award is given in two equal installments – one in September 2009, the other in January 2010. If you are an MBP student, Carolynn Mar will send you an email in late August indicating the amount of your OSOTF award. If you are not an MBP student, you can either email Carolynn (include your supervisor‟s name) in late August to request the amount or ask your supervisor‟s administrator for the amount of your award. Students starting their program in September 2009 (1st year students) will not be considered for funding during this round – there will be another competition for 1st year students in September 2009 with the awards given out in January 2010.
