Request for access to Corporate Information Systems Essential information for all applicants. 1. Call log number You must have a log number from Customer Services (x3800). Please enter it in the Customer Call Log Number box at the top right of the form. 2. Staff account You must have a working University staff domain account (Staff image) before this application can be processed. If you do not have one, you must apply by completing form IT89 available from Information Services. Applications for staff PC accounts must be returned to: Server Applications Support, Information Services, Octagon, Beaconside. 3. Please complete all sections of the form that apply to you. Incomplete forms will be returned. 4. Information Services will notify you of your username and password. 5. Authorisation This form must be authorised by the appropriate person in your Faculty or Service – normally the Director of Faculty/Service or Business Manager. The authorisation signature will be verified against a list held by Information Services. 6. Please return the completed form to the address shown at the bottom of the form.
