   Goal
       Teach beginning nursing students functional
        assessment of an older adult
   Objectives
     Students will describe common components of
      standardized functional assessment
      instruments.
     Students will demonstrate one component of
      functional assessment, ie. Get Up & Go Test
       Goals of Functional
       Assessment
 Monitor   function in older
  adults
 Prevent decline
 Maintain function of the elderly
 Decrease   risk of falls
 Plan for future care needs
 Establish baseline functional status
 Repeated over time to assess
  changes in status
          Common components of standardized
          functional assessment tools

   Sensory
     Vision - Reading
     Hearing – whisper test

   Cognitive
       Mini-cog
   Ambulation
       Get Up & Go Test
             Get Up & Go Test

   Acute care, outpatient, community settings
   Timed or untimed
   Assess stand, walk, pivot, sit, risk for falls
   Scoring
       Freely mobile  impaired mobility
       Hendrich II Fall Risk Assessment
            rating (0-4) depending on assistance needed to get up from
             chair
   Basis for interventions
       Ambulation, PT/OT consults, assistive devices
         Timed Get Up & Go Test
         Instructions
   The person may wear their usual footwear and
    use their usual assistive devices
    Have person sit in a chair with back against
    back of chair and arms resting on arm rests
   Have person stand up and walk 10 feet, turn
    around, walk back to the chair and sit again
   Timing begins when person begins to rise from
    chair and ends when person returns to chair and
    sits down
   The person should be given one trial and then
    three timed tests
   Average the times of the three timed tests
   Rating
     < 10 sec: Freely mobile – normal balance,
      steady gait
     < 20 sec: Mostly independent
     > 20 sec: Impaired mobility – clear
                  evidence of fall risk

         Instructions
