1) You have a chance to win one of two $25 Fannie May gift cards!
   Each person who returns a completed order form will be entered in the
   drawing. Winners will be notified after all orders are in.

2) This year all your candy orders will be delivered DIRECTLY TO YOU. On the
   candy order form, press hard to ensure writing comes through the 3 copies.
   In the “Ship T o” section, please print your name/address/phone
   information. Your candy order can be sent to either your home or place of
   employment. If the order is to be delivered to your workplace, include the
   company name on the address line. In the product section area, make sure
   you are using the price in the “your price” row, which includes the 10%
   discount. Fill in the totals on the bottom and right side of the form. Keep the
   pink copy for your records.

3) The Gift Card form is separate this year. Since the gift cards will be delivered
   to the high school, in the “Ship to” section, just fill out your name and phone
   number. Gift card pick up will be in the high school cafeteria Monday,
   December 14th from 4:30 – 6:00 PM.

4) Payment must accompany your order. Make sure all checks are made out to
   “H.E.L.P.” If anyone is ordering both candy and gift cards, please write two
   separate checks.

5) Please mail forms and checks to me by Friday, November 13th . Your order
   will be delivered to you the week of December 7th . You can also leave your
   order in an envelope addressed to “Joan Gebbia/Fannie May” in the school
