                                    24th Annual Schedule 2 Employers’ Group Conference
                                      October 8-9, 2013 | Sheraton Parkway Hotel, Richmond Hill ON

                                                               Application Deadline: May 31, 2013

                      IMPORTANT – PLEASE READ – Submission of this application constitutes acceptance of the following Terms & Conditions:

Applications are subject to review and approval by the Conference Planning Committee of the Schedule 2 Employers’ Group (S2E Group) and will be reviewed in the
order they are received. Only approved applicants will be notified.

Sessions are 2 hours in length (including Q&A). Speakers are required to provide adequate material to speak for the full length of the session. Approved applicants are
expected to fulfill their commitment to deliver a presentation exactly as described in the application (or to arrange for a qualified substitute, if needed). S2E Group
reserves the right to edit session titles and descriptions for publication online or in print. Submission of this Application constitutes permission for S2E Group to
publish the information contained in it either in printed or electronic format. For approved presentations, a copy of your presentation slides must be submitted no
later than Friday, August 30, 2013.

Speakers are required to provide their own laptop computer. A screen, projector and podium (with microphone, if needed) will be provided by S2E Group. Other
equipment, if needed, can be arranged for a rental fee.

For approved presentations, S2E Group will provide complimentary 2-day Conference admission for a MAXIMUM of 2 speakers per session. Additional participants
(whether co-presenting or providing administrative assistance) are allowed but will be required to pay the regular registration fee to gain admission to the
conference/event and will not be named on the official Conference Agenda.
