To apply for membership today, call 800.CDA.SMILE (232.7645). How do I receive C.E. credit for attending a course? Please see On-Site Guide for details. How can I contact someone attending the convention? CDA will have an electronic message center available in the registration area for attendees to retrieve e-mail, check phone messages, and send and retrieve electronic messages to and from attendees. What if I lose my badge? Please hang on to your badge. There is a $10 on-site replacement fee. If you do lose it, you will need to go to the Badge Correction Booth in the Registration area. When will I receive my badges? You should have received your advance registration materials within four weeks of submitting your advance registration form. (Note: Badge mailing began on June 16 for forms received prior to this date.) This includes anyone registering by mail, fax or online. Will there be a coat/baggage check available? A coat/baggage/stroller check will be available in the lobby of the Moscone West Convention Center for $2 per item. Are children permitted in the Exhibit Hall and lectures? For the safety and convenience of all attendees, children younger than 13 will only be permitted in the Exhibit Hall from 9 to 11:30 a.m. daily. What if I have a question regarding registration or if there is a problem after the advance registration materials are received? Simply call CDA’s Resource Center toll-free at 800.CDA.SMILE (232.7645). What do I need to bring to register on site? Photo I.D. and, if applicable, your ADA card, student I.D. or Dental Board allied dental health professionals license. What if I register after the Aug. 2 deadline? Anyone not registered by the Aug. 2 deadline will have to register on site. There is a $20 on-site registration fee for staff and guests. What is CDA’s cancellation policy? Prior to Aug. 9, 2006, cancellations must be made in writing and include accurate recipient information. All materials should be returned to the California Dental Association address on the registration form. After Aug. 9, 2006, refunds will not be given. How do I correct a misspelled badge? Fax a copy of the badge to (916) 498-6178 with the correction by Aug. 9, 2006, and we will send a corrected badge. You can also go to the Badge Correction Booth in the on-site registration area at the Moscone West Convention Center. How can I attend a workshop? Tickets to fee workshops were available for purchase in advance by using the advance registration form before the Aug. 2 deadline.
