LIST TIMES AVAIL.: This contract, when signed by both parties, is an agreement between an independent contract delivery person (Driver) and Plaza Flowers (Plaza). Driver accepts the following requirements as the minimum acceptable conditions of agreement: (1) (2) (3) (4) (5) (6) (7) Driver does possess a valid vehicle driver's license. Driver provides a fully insured vehicle with valid registration & inspection. Driver accepts full accident liability for personal injury and vehicular damage. Driver accepts full responsibility for any traffic violations. Driver will conduct him/herself in a manner becoming of a professional. Driver will obtain signatures for all deliveries (recipient, neighbor, coworker, nurse, volunteer, guard, etc.) Driver will deliver all products in the same condition as they left the shop. ABSOLUTELY NO SMOKING IN VEHICLE.
Plaza and Driver accept the following conditions of compensation: (1) 5 business days after completion of deliveries, Plaza will pay Driver for each and every delivery completed in good condition to the correct address at the correct time. Each address is considered one delivery regardless of the amount of packages. (2) The compensation is four dollars ($4.00) for every delivery in 19403; five dollars ($5.00) 19087, 19401, 19404, 19405, 19406, 19407, 19408, 19409, 19420, 19422, 19426, 19428, 19456, 19460, 19462, & 19481; seven dollars ($7.00) for every delivery in the remaining zip codes. It is hereby acknowledged that Plaza Flowers is not liable for personal injury or vehicular damage incurred while Driver is performing duties as a contract delivery person. ______________________________________ Plaza Flowers Management DATE: _________________________

Independent Delivery Contractor Instructions
DECIDE: 1. What area do I know best? (Plaza Flowers deliveries to the following counties: Philadelphia, Montgomery, Chester, & Delaware. You choose the area.) 2. What vehicle should I use? (The less times you need to reload, the more money you earn.) ITEMS NEEDED: 1. 2. 3. 4. 5. 6. 7. WHEN: 1. Unless you are instructed otherwise, arrive at 8 AM on dates you indicated on contract at 417 Egypt Road, Norristown. 2. Expect to deliver late, although you choose your hours. 3. Pick up payment for all successful deliveries on 2/18. PROCEDURE: 1. Look up all unknown addresses. Write map coordinates. 2. Organize route in most efficient order. 3. Verify any questionable or unlisted addresses. 4. Phone all hospital deliveries to make sure patient can receive flowers. 5. Highlight or circle time deliveries. 6. Load deliveries securely in vehicle. Remember, you only get paid for non-damaged deliveries. 7. Double check count. Verify order numbers. 8. Check for multiple pieces. If you forget part of a delivery don't get paid for it. 9. Get dispatcher's approval, then depart. 10. Obtain a signature from each stop - can be a coworker, receptionist, nurse, etc. (indicate on sheet). 11. If not home, leave with a neighbor. Tag the door of the recipient so they know where the delivery was left. Write order number on all door tags. 12. Call 1-888-800-8826 ext 108 if you need assistance. Remember you don't get paid for deliveries that you bring back. 13. Return to shop as quickly as possible for next trip. Comfortable running/walking shoes. Heated vehicle - The larger, The better.
