DIASTOLIC HF Rx: - The DIG ancillary trial, a parallel study to the DIG trial, evaluated the role of digoxin in patients with HF and an LVEF >45 percent. At a mean follow-up of 37 months, digoxin had no effect on all-cause or causespecific mortality, or all-cause or cardiovascular hospitalization - A '93 M/anal showed more LV reversal w ARBs (13%) > CaCB > ACEi > Diur > BB (6%). The clinical signif of diff is unknown. - However, the largest RCT to date did not show any benefit w ARB's beyond BP reduction (Effect of angiotensin receptor blockade and antihypertensive drugs on diastolic function in patients with hypertension and diastolic dysfunction: a randomised trial. Lancet 07 23;369:2079). - Verapamil is reported to particularly good lusitropic properties. - The benefits of ACEi could be explained by a reduction in myocardial angiotensin II production.
