Fill out adoption application. This form should be returned to HCHS along with adoption fee. Adoption packet includes adoption agreement, veterinarian certificate if animal is not sterilized, and a listing of participating veterinarians for the adopter’s records. The certificate covers the cost of the rabies vaccine and spay/neuter of your pet. Veterinarian certificate should be taken with the animal to your vet, or participating veterinarians. The certificate needs to be returned to HCHS by the veterinarian after the pet has been examined, spayed or neutered by the date on the certificate. This is very important! Helpful Tips for the New Pet Parents

Be prepared to bring your new pet home, for a dog you will need a collar and leash, for a cat you will need a litter box and cat litter, and for either you will need food and water bowls, and of course, some toys. And don’t forget to order an identification tag right away. Cat’s value cleanliness, so it is best to clean their box daily or several times a week to make sure it is up to your new kitties standards. Dogs need order; they are pack animals, so make yourself the “pack leader”. Let your new pet know from the start who is the boss. Assume your new dog is not housetrained, and work from there. Read over housetraining information. Be consistent, and maintain a routine. Dogs need an active life so you should plan plenty of exercise and game time for your pet. Finally, be reasonable in your expectations. Life with you is a different experience for your new companion, so give them time to adjust. You’ll soon find out that you’ve make a friend for life. Humane Society of the US has a website – Pets for Life with lots of useful information for pet owners. PetSmart offers obedience classes with discounts for adopters that you may want to take advantage of. This may make life with your new pet even more enjoyable. ADOPTION AGREEMENT (This is a legal document. Please read carefully) I hereby adopt this animal in its present condition and understand that the Hart County Humane Society is not responsible for any medical condition not readily detected prior to or at the time of adoption. I promise not to use this animal for vivisection, breeding, or in any inhumane way. I will have this animal sterilized in accordance with the spay/neuter policy of the State of Georgia Code 4-14-1. I will not allow this animal to breed. If I do, this agreement with the shelter is void and I understand that I will be subject to fines up to $2,000. In the event that this animal is transferred to another person, I will be responsible for notifying the Humane Society with the new owner’s name. The new owner is responsible for getting the animal sterilized in accordance with the State of Georgia law. I agree to provide adequate care and observe all county and state animal regulations. I agree to provide proper veterinary care for this animal. I agree to keep this animal as a pet and companion animal and not to use it for any other purpose. If my new companion is a dog I agree it will not be tethered more than eight hours a day and will always be provided shelter. If my new companion is a cat it will not be allowed outside unsupervised in a high traffic area and will always be provided shelter I agree to provide adequate food, fresh water, and socialization. Thousands of healthy animals are euthanized every week across this country, while others are left to fend for themselves without adequate food or shelter. This suffering is caused by the over population of companion animals due to irresponsible pet owners. The Hart County Humane Society is dedicated to making this a distant memory. All of our pets are sterilized before adoption if they are old enough. If you adopt a pet that is too young the adoption fee pays for a certificate to take to the veterinarian to have the surgery. With the certificate is a list of veterinarians that have agreed to do the surgery and rabies vaccine at no additional charge to you. You can take the certificate to any other vet and we will reimburse them $55 for a dog and $45 for a cat. You have signed a legal contract stating that you will do this. If we do not receive the certificate back within two weeks on the date of your contract we will be forced to come get the pet. Please help us be a part of the solution and not the problem. Not only is it better for the community if your pet is sterilized it is also better for the health of your friend. Cats and kittens have been tested for leukemia and FIV before adoption. Dogs over four months have been heartworm tested, both have had at least one round of shots and have been de-wormed. You are responsible for any additional vaccinations that your new pet may need. They have agreed to accept our certificate as payment for the routine spaying or neutering and rabies vaccine for your new pet. Your pet has received at least one set of vaccinations while in foster care. He or she has been treated for internal parasites, ear mites, and fleas. All dogs older than 4 months have been tested for heartworms and put on a preventative. All cats and kittens have tested negative for feline leukemia and FIV. You can also take this certificate to your own veterinarian if he or she is not on this list. Most will honor it as a rescue animal. Upon completion of the surgery have the doctor sign the certificate and mail it to the address below and we will reimburse them $55.00 for dogs and $45.00 for cats (any extra charges are your responsibility). Your new pet may be due for future vaccinations that are not included in the adoption fee. This will be your responsibility for the health of your pet. The Hart County Humane Society wants to thank you for giving a homeless animal a family. We wish you and your new friend many happy, healthy years together.
