MODEL RELEASE
A Model Release is a form of release that is signed by a model. This release usually releases a photographer or company from all liabilities in the photographer’s/company’s use of the model’s image/picture and grants permission to the photograph to use of the picture/image in the media such as magazines, catalogues, advertisements etc.

MODEL RELEASE
FOR GOOD AND VALUABLE CONSIDERATION, the undersigned (hereinafter referred to as the “Model”) hereby assigns and grants permission to _____________________ (hereinafter referred to as the “Photographer”) to license and publish all photographs and images of the Model, including the use of the Model’s name and stage name, in the media for any purpose which may include but not limited to, advertising, promotion, marketing, packaging for any product or service or any other lawful purposes. THE UNDERSIGNED hereby, in good and valuable consideration, the receipt and adequacy of which is hereby acknowledged, do hereby remise, release and forever discharge the Photograph and each of its/his/her successors and assigns, from all manner of actions, causes of action, suits, debts, liens, dues, accounts, bonds, covenants, contracts, claims and demands whatsoever which heretofore may have been or may hereafter be maintained by the Model, directly or indirectly, as a result of any matter whatsoever; THE MODEL further agrees not to make claim, sue or continue suit against any other person
