									Surgicom-ASSA Practice Cost Study 2009 for 2010
Dear Colleague
Too few returns were received last year and our submission to Dept of Health was not accepted on the basis of too small a dataset. We are contesting this in a complex legal battle under the auspices of SAPPF and Healthman. Additional data is urgently needed to bolster a new 2009 submission for 2010.

We need your URGENT co-operation:
1. Resend your previous submissions; many were NOT received by Healthman Check on last page to see if your data was received

2. If not yet sent, PLEASE CO-OPERATE AND SEND ASAP Ask you business manager/accountant to complete the questionnaire.
