					STATE OF NEVADA AGING AND DISABILITY SERVICES DIVISION SERVICE SPECIFICATIONS PODIATRY SCREENING
Any exception to these Service Specifications must be requested in advance in writing and approved by the Deputy Administrator. SERVICE DEFINITION: This service provides an annual foot screening and referral services. SERVICE CATEGORIES AND UNIT MEASURES: Podiatry Screening: provides seniors with comprehensive foot exam as well as appropriate referrals and instructions on proper foot care. Also includes toenail clipping, etc., if a senior has difficulty performing these tasks. One unit equals one foot screening visit. GENERAL REQUIREMENTS: A. Pursuant to NRS 632.010-632.500, grantees must meet all applicable statutes pertaining to nursing.

SPECIFICATIONS: 1. Required Services: 1.1 Provide patients with the opportunity to receive an annual foot exam: the exam must include at a minimum a general assessment of feet, toenails, circulation and referrals to a specialist, when needed. Provide patients with instruction on proper foot and nail care. Trim toenails if client has difficulty reaching them, has poor hand strength, etc.

Documentation Requirements: 2.1 Establish individual patient charts indicating the results of the foot exam, other referrals provided and follow-up on referrals.

Operating Procedures: 3.1 Establish a scheduling system that maximizes utilization of available patient appointments.

Service Specifications Podiatry Screening Page 2 of 2 3.2 Maintain an appropriate referral listing and provide referral assistance to all patients who require medical consultation and care beyond the scope of services offered by the program. Establish written protocols for the health services provided by the Certified Nursing Assistants (CNAs) under the supervision of the Podiatry Consultant and clinic supervisor.

Training: 4.1 Podiatry Consultant will provide an annual comprehensive training on foot examination and identification of problems to CNAs. Podiatry Consultant will provide annual instruction on how to properly prepare and cut nails. Podiatry consultant or nursing supervisor will evaluate CNA's skills at a minimum of twice yearly.

Quality Improvement: 5.1 Conduct at least one quality assurance review annually. The review must evaluate the quality of medical service provided by the program and the adequacy of documentation. The results of the review must document any program deficiencies and contain a plan of correction.
